    本研究は小児における医療関連感染予防対策の取り組みとして、これまで明らかにしてきた入院中のがん患児及び潰瘍性大腸炎、クローン病等の炎症性腸疾患（IBD）患児のClostridioides difficile 消化管保有の検討をもとに、在宅療養中のIBD患児のC. difficile 感染予防の実践状況を明らかにし、C. difficile感染の減少およびIBD患児のQOLの向上を目指した支援方法の構築を行うことを目的とする。 令和4（2022）年度は、三重大学医学部附属病院およびあいち小児保健医療総合センターに入院・通院中の5～15歳の炎症性腸疾患患児（潰瘍性大腸炎およびクローン病）を対象に C. difficile 消化管保有の実態調査を行ってきた。既存の研究からの継続的なデータ収集であるが、これまでに19名の患児からの継続調査を行っているが、今年度は新規患者を得ることができなかった。また、19名中18名は潰瘍性大腸炎の患児であり、クローン病患児の対象が少ない状況である。さらに、小児が通う幼稚園や学校等において、排泄ケア環境の実際や患児及び学校関係者（教諭等）へのインタビュー調査を予定しており、その準備として、研究分担者や研究協力者との調整および三重大学医学部での研究倫理審査委員会への申請を行なった。 令和5（2023）年度には、IBD患児へのインタビューやIBD患児が通う学校関係者へのインタビューおよび学校へ出向いた調査を実施できるように準備を行う予定としている。

    本研究は，結核感染予防のために構築した医療施設職員の結核健診管理・教育システムを臨床応用し，医療施設で主体的に実施されている職員の接触者健診を評価し，施設職員の健康を継続支援することを目的としている。 令和4年度は，モデル地域とする東海3県（愛知県・岐阜県・三重県）の感染管理認定看護師へ施設内での職員の結核接触者健診についての質問紙調査を実施し，同時に開発した職員接触者健診データ管理用ソフトウエア（開発したソフトウエア）を試用してもらう施設を募った。質問紙調査は28名からの回答を得た。その中の3施設の施設長から承認，感染管理認定看護師からの同意を得て，開発したソフトウエアの試用を開始した。試用開始前に，3施設の感染管理認定看護師へ，職員接触者健診のデータ管理状況や課題等について事前調査を実施した。その後，各施設で発生した実際の結核接触者健診のデータを開発したソフトウエアを用いて管理を開始してもらった。試用期間中に使用感や管理状況についての調査を行う予定で進めている。 令和3年に検討したソフトウエアの設計上の問題については，データ一括入力機能や項目の整備，細かな修正点を含め改訂を行い，現在テスト段階である。令和5年には，開発したソフトウエアを試用している3施設のソフトウエアを改訂版に更新し，試用を継続してもらう予定をしている。

    感染症患者に対する看護師の倫理的行動および倫理的行動に影響する要因について、感染症看護専門看護師（以下CNS）および感染管理認定看護師（以下ICN）、看護管理者、感染症病棟の看護師を機縁法でリクルートしインタビューを実施した。 対象者は総計19名で、勤務地は東北、関東、中部、近畿地方であった。CNSは3名、ICNは4名で、看護師平均年数は24年、CNSまたはICNとしての勤務平均年数は9.6年であった。看護管理者は5名で、看護部長（局長）経験平均年数は3.8年であった。感染症病棟の看護師は7名で、看護師経験平均年数は21.1年、感染症病棟勤務の平均年数は4.1年であった。インタビュー内容は、感染症患者の看護で大切にしていること（必要な配慮）、倫理上の問題とその要因等で、リモートでの半構造化面接法を実施した。録音データから逐語録を作成し、文脈を読みとって切片化したコードから意味内容が類似しているものを集め、帰納的に＜サブカテゴリ＞、≪カテゴリ≫、【コアカテゴリ】へと抽象化し分析した。 感染症病棟の看護師の倫理的行動については、【感染症患者に必要と考えて実践した看護】、【感染症看護の実践を支えるもの】の2コアカテゴリ、≪患者・家族への精神的ケア≫、≪感染症看護を支える規範≫など5カテゴリ、<面会できない患者のために家族とつないだ>、＜閉鎖環境に置かれた患者への心のケアを重視した＞等、19サブカテゴリが抽出された。倫理的課題については、【感染症看護における倫理的課題】、【看護師の倫理的行動を妨げるもの】の2コアカテゴリ、≪感染防止策が患者の利益に反することへのジレンマ≫、≪問題ある看護師自身の行動≫など5カテゴリ、＜行動制限で運動ができず、ADLが低下するのをもどかしく思った＞、＜自分自身が感染していないか不安だった＞等、29サブカテゴリが抽出された。

    本研究は，新生児GBS感染症予防のために，新生児GBS感染症の発症要因を明らかにすることおよび新生児敗血症髄膜炎発症GBS株と非発症GBS株の微生物学的な異同を明らかにすること（第一研究），GBS保菌妊産褥婦への予防対策とケアを明らかにすること（第二研究）を目的としている． 平成30年度は，現在までの成果を踏まえ，研究方法について共同研究者および研究協力者と具体的な研究方法について検討を重ね，研究精度の向上を図った．第一研究は，多施設における早発型GBS感染症および遅発型GBS感染症の発症状況と発症頻度を明らかにすることを継続して実施することとした．細菌学的解析は，血清型，薬剤感受性，MLST型，CovR/S変異を解析することとした．所属の研究倫理委員会の承認を得て，研究協力施設5施設の内諾は得られ，4施設においては施設の倫理委員会の承認が得られ継続実施できている．新生児GBS感染症7症例の情報収集を行い，研究対象者から検出されたGBS菌株7株を収集した．薬剤感受性においてはpenicillin系抗菌薬に耐性を示した株はなく，血清型はIII型6株，Ib型2株であった．今後は，研究対象施設数の増加を図り，対象者の情報収集および細菌学的解析を実施していく． 第二研究は，現在までに実施したGBS保菌妊産褥婦を対象とした面接調査の結果を発展させ，GBS保菌妊産褥婦へのケアについて検討した．今後は，新生児GBS感染症予防のためにどのような予防対策が講じられているのかに関する調査内容を決定し，調査を開始する．

    In inflammatory bowel disease(IBD) children ( ulcerative colitis ;UC and Crohn's disease;CD) aged 5 to 15 years, fecal toxin detection, Clostridioides difficile (C. difficile) isolation culture, identification of toxin production patterns in isolated strains, and analysis by Polymerase chain reaction were performed. As a result, C. difficile was isolated from 6 of 19 (31.6%) and 30 of 113 fecal samples (26.5%).

    This study aimed to clarify the current state of and issues related to continuing support for hospital staff who receive tuberculosis (TB) examinations for TB prevention at medical facilities and to develop software that will provide continuing support. Interviews regarding TB examinations were conducted with 10 nurses certified in infection control who had experience with TB contact examinations at medical facilities in Japan. Analysis of the interview contents revealed that TB examination data of staff were not being converted into data that can be used to manage their health. To address this, we designed software using Microsoft Access 2016 to manage information of people infected with TB and linking it with information of staff who undergo contact examinations. The software allows for managing the history of staff who undergo multiple contact examinations during their employment, and will continue to be improved for practical use through its repeated use at medical facilities.

    Ten people with persistent hepatitis B virus infection (HBV carriers) were interviewed regarding their experiences with medical facilities. From the interview contents, four core categories (negative experiences, positive experiences, social support, and effects of disease) were extracted. A subsequent questionnaire survey was administered to 1,152 HBV carriers to compare their experiences at the time of diagnosis and their most recent interaction with healthcare personnel. The results revealed improvements in discrimination against them, doctors’ explanation of the disease, and support after being informed of the diagnosis. However, the behavior of nurses did not significantly improve, highlighting the need to improve nursing support for such patients.

    In the past several years, Japan has been promoting initiatives to establish a “community-based integrated care system” in order to support its aging population. Clean intermittent catheterization (CIC) is a technique that helps elderly individuals to perform self-care by enabling them to urinate independently at home. However, CIC is not a simple technique for the elderly to learn given the challenges they often face, such as reduced visual acuity and manual dexterity and the inherent difficulty of modifying lifestyle habits formed over many years. With this in mind, we interviewed CIC patients aged ≧65 years to investigate nursing techniques that support the continued use of CIC either at home or in aged care facilities. The interview responses highlighted a range of issues, including circumstantial accounts of CIC performed at home. In the present study, we describe a single case drawn from these interviews.

    This study was conducted at three hospitals from May 2016 to March 2019. 104 patients were included in this study. The dialysis patient was in an easily infected state (ALB2.7, Hb8.6), and due to acute exacerbation of the disease, ADL decreased and clean skin care was not performed, and a catheter insertion procedure was performed. The doctors' compliance rate for infection prevention behavior was 84.8% for hand hygiene and 100.0% for wearing masks / sterile gloves, suggesting thorough adherence to MBP when the surgeon changes urgently. Skin-resident bacterial species accounted for 80.0% of the detected bacteria. There was one case of infection. Critical preventive measures include clean care to reduce bacteria on the patient’s skin before the placement of the catheter for temporary hemodialysis, thorough skin disinfection at the catheter insertion site, adherence to maximal sterile barrier precautions.

    The subjects were children who developed early-onset GBS disease (EOGBSD) or late-onset GBS disease (LOGBSD) and their mothers from 5 institutes between 2007 and 2016. There were 14 cases of EOGBSD and 17 cases of LOGBSD, inborn cases were 4 cases both EOGBSD and LOGBSD, with estimated incidence of 0.10 per 1,000 live births for both EOGBSD and LOGBSD. We divided the study period into two parts; the period before (period I: 2007-2008) and after (period II: 2009-2016) issue of the guideline for prevention of vertical transmission in Japan. The incidence of EOGBS was significantly lower in period II (0.03 per 1,000 births) than in period I (0.42 per 1,000 births) (p = 0.02). The incidence of EOGBSD would further decline with proper timing of GBS screening, sampling site, culture method, and appropriate antibacterial prophylaxis for pregnant GBS carriers.

    The purpose of this study was molecular epidemiological analyses of isolated strains of antibiotic-resistant bacteria in a prefecture of Japan and finding a better strategy for making effective progress in improving infection control in hospitals with fewer than 400 beds. Analyses found persistent infections brought into the hospitals that were community-acquired, antibiotic-resistant bacteria that had community-specific genotypes and antibiotic-resistant genes that had the potential to propagate via bacterial plasmids. Improvements in effective infection control in the hospitals occurred formally, such as via infection control committees or informally from the ideas of staff members wanting to provide higher quality healthcare. The results suggested that an important strategy for improving infection control is to discover positive deviance which are effective staff member behaviors and formalize them.

    We evaluated healthcare-associated infections and their prevention measures in outpatient services. We conducted a survey targeting 428 outpatient staffs (medical/non-medical staffs) at 3 hospitals in Prefecture A. We measured antibody titers for measles and other infections and investigated the implementation status of infection control measures. Our analysis of the results revealed that better education was needed for the staffs to understand the need for vaccination. We then conducted a survey targeting 1,369 medical institutions nationwide and obtained response from 570 institutions (41.6%). From analysis, we found that influenza (84.7%) and tuberculosis (75.4%) were needed to conduct the infection control. The protective equipment such as masks was prepared enough. However, there were 34.1% of the medical institutions installing depressurized rooms, and 31.3% installing sputum sampling booths. Infection control measures in outpatient services therefore need further improvement.

    We conducted an interview survey and questionnaire survey about the problem of communicating information relating to infections between visiting nursing stations and clinics, and the state of measures for dealing with infections at clinics which collaborate with visiting nursing stations. The data obtained from visiting nurses suggested that when different chief physicians gave instructions about the same procedure, there were differences in details, such as the items supplied and disposal methods, and also that clinics had low awareness about the prevention of occupational infections. Few visiting nurses used portable needle disposal devices, which are an effective measure for preventing needle sticks. We worked to improve this inconvenience and carry out development based on a foundation of safety.

    The CRBSI-related factors ,significant differences were observed as well as the hemoglobin (Hb) (p < 0.01)and albumin (Alb) values (p < 0.001).Staphylococcus hominis was detected in the blood and the skin of PFGE patterns (Similarity 100%). Staphylococcus epidermidis was detected in the skin on the three times of PFGE patterns (Similarity 100%). The homology between the skin and the blood of PFGE patterns (Similarity 42.9%). It is important to complete the skin disinfection of the patients, hand hygiene and conduct maximal sterile barrier precautions.

    The outpatient unit is associated with a high risk of infection because it is an area where many patients and hospital staff converge. We administered a self-report questionnaire on topics such as fever and cough to 23,065 outpatients(valid response rate, 87.0%), and analyzed the responses in reference to the patient diagnoses. The results showed that 78.6% of patients with infections had diseases requiring measures against droplet infection. We performed antibody measurements for vaccine-preventable diseases(VPD) such as measles and rubella and conducted a questionnaire survey on infection prevention in a total of 324 staff members ; they revealed that some staff members continued to engage in duties without acquiring immunity, indicating the need for vaccine programs.

    Taken together, it is necessary to share information about drug-resistant bacterial infections between medical facilities and home-visit nursing care stations to construct the system preventing transmission of drug-resistant bacteria conducted in regional areas. Coordination and cooperation between experts in infection control of administrative offices and medical facilities are also required to establish an educational program and infection control measure improving the quality of practical cares in medical facilities.

    A survey was conducted on adverse events as well as compliance with standard precautions at home visit facilities, using a semi-structured interview and a questionnaire. The survey of home visit facilities across Japan showed that had infections as adverse events were reported by 77 managers (39.7%) out of the 194, and 97 nurses (18.7%) out of the 520 who returned the questionnaire. The most frequent adverse event was needlestick injury. Other frequent adverse events were infections with scabies or tuberculosis, and norovirus. It was suggested that the cause of adverse events relating to infection are connected to the current situation where information on infection and safety precautions is hard to obtain, as well as a lack of mastery of standard precautions. We need to take steps for prevent the adverse events relating to infection in the provision of visiting-nurse services.

    2007年度の計画と経過 : 看護師が行う感染予防のための遮断技術を検討するために, 集中治療室[以下ICU]で多く検出されるメチシリン耐性黄色ブドウ球菌[以下MRSA]を指標として, 人工呼吸器を装着した患者に行う気管吸引操作時の看護技術と療養環境の汚染に注目し次のことを行った. (1) 気管吸引時の看護師の感染予防行動を観察して気管吸引操作前の不適切操作や気管吸引操作終了後の療養環境と医療器具の高頻度接触部位を明らかにする. (2) 臨床における患者のMRSAの有無別にみた療養環境からの環境検出菌を明らかにする. その結果, 40人の人工呼吸器装着患者100場面の観察から, 気管吸引操作終了後の高頻度接触部位はリネン, ベッド柵, モニター/ポンプボタン等であった. 気管吸引を必要とする患者の療養環境の細菌検索から60検体中15検体(5.8%)からMRSA が検出された(リネン, 聴診器, ジャクソンリース, 人工呼吸器の消音ボタン). その中の2検体(0.8%)は, MRSA感染者ではない患者の療養環境からの検体であった. 2008年度の計画と経過 : 本学と同程度の教育環境にあるイギリスの看護系大学を選定した後に訪問して, イギリスにおける看護基礎教育における感染看護の実態と教育教材の収集を行い効果的な感染看護の教育方法の示唆を得る. 2007年度に本学の教育をまとめたものを踏まえOxford Brookes大学を訪問し, 収集した資料をまとめ成果発表を行った.

    The purpose of this research is the economic evaluation of the nursing service in a cancer patient by the amount of nursing estimated to be needed with a quantification model. We investigated the amount of nursing services of the family, the nurse, the helper and the friend / acquaintance about a cancer patient's nursing services. 183 cancer patients of a university hospital general ward and 52 cancer patients in a palliative care unit, whose consent and cooperation were obtained to this research, were investigated about the amount of nursing services of a nurse and a family. The nurse carried out by the self-entry system, and the family carried out with the interviewing method by a nurse. As a result, the nurse judged that it was insufficient about nursing of a university hospital general ward cancer patient. The family also judged that it was insufficient about "daily attendant care" and "counseling and guidance". However, judgment of both in "assistance in medical treatment" was not in agreement. In a palliative care unit cancer patient, the judgments of the nurse and the family were insufficient about "daily attendant care" and "counseling and guidance". For a university hospital general ward cancer patient's family, "counseling and guidance" and "help with household chores" were burdens. However, a palliative care unit cancer patient's family wanted to carry out not a burden but further nursing. We are analyzing these results including the data of the amount of nursing care of the home cancer patient who is precedence research, from many sides for instance needs / demands. We are tackling the economic evaluation of the nursing service to medical-treatment / nursing and caregiving systems configuration which supports Quality of Life of a cancer patient and a family.
