    抗コリン薬による腎での水吸収に関して、0.9％生理食塩水を静脈内投与して利尿状態をラットに作成した。投与開始2時間後各種薬剤（抗コリン薬3種類、desmopressin）を投与した。尿量モニターとともに尿中Aquaporin-2 (AQP2)蛋白濃度をELISA法で測定、最後に摘出腎を皮質と髄質に分け、AQP2の蛍光免疫染色を行い、またcAMP量をELISA法にて測定した。vehicle(VE)投与群では尿産生のピークが出現したが、imidafenasin (IM)、atropine(AT)、tolterodine、desmopressin(dDAVP)投与群では用量依存性に尿産生が抑制された。摘出腎の皮質では、IMとdDAVPによりcAMPは有意に増加した。AQP2免疫染色において、利尿状態ではAQP2分子は腎皮質集合管の細胞質内に分布したが、IMとdDAVP投与により集合管管腔側へ移動traffickingが認められた。さらに尿中AQP2蛋白の排泄量はIMとdDAVP投与により増加がみられた。尿中のNa+排泄量を測定するとIM高用量で尿Na+は低下した。すなわち、IMにより腎でNa+の再吸収が起こっている可能性が示唆された。 腎集合管にはムスカリン受容体（M1サブタイプ）が存在すると報告されているが、本結果から抗コリン薬はM1受容体遮断を介して腎皮質cAMPの上昇をきたし、集合管AQP2のtraffickingから尿量減少が生じたと推測している。生理食塩水負荷により腎皮質からacetylcholine (ACh)が放出され、AChがepithelial sodium channel (ENaC)のNa+吸収を阻害しNa利尿に働くと報告されている。抗コリン薬はENaCを介してNa+吸収を促進した可能性もあると考えられる。

    Clinical research and basic experiments in our laboratory have revealed that anticholinergics reduce urine output at night. Recent reports suggested a mechanism of urine absorption through the bladder epithelium, but at most about 10% of the infused volume. In the present study, bladder absorption rate depended on the intravesical pressure and the infused volume, furthermore, the anticholinergics was not involved in bladder absorption mechanisms. Next, an experimental system was constructed to measure the amount of urine directly from the ureter after intravenous administration of physiological saline to make rats a diuretic state. The anticholinergics suppressed renal urine production, and the higher the dose, the stronger the antidiuretic effect. Water reabsorption by AQP2 molecules was speculated by elevated urinary sodium concentration. Immunostaining of the renal collecting duct was performed using an antibody against AQP2, and it was confirmed that AQP2 was indeed expressed.

    We evaluated the natural history of nocturia and among Japanese men and women in a longitudinal study. Nocturia was defined as two or more voids per night. The present results showed fluctuations in nocturia during the 5-year period. These fluctuations might have been due to the multifactorial etiology of nocturia, including aging, lifestyle, obesity, hypertension and diabetes. We also analyzed the relationship between overactive bladder (urgency) and age, gender, metabolic syndrome (MetS) among participants who had nocturia. The logistic regression modeling were used for statistical analyses. A significant association was found between nocturia with urgency and MetS. The age and sex-adjusted odds ratio (95%CI) was 1.49 (1.22-1.82). Our study confirmed that individuals who report nocturia with urgency are more at risk of MetS than those with nocturia without urgency. The combination of symptoms should prompt closer attention to cardiovascular health among primary care providers.

    Antimuscarinics have the potential to decrease nocturnal urine volume via suppression of bladder afferent C-fibers. Can other antimuscarinics and α-adrenergic blocker decrease urine production? Intravenous α blocker naftopidil decreased the urine production in rats with diuresis. This effect was found to depend on its inhibitory influences on C-fibers. We examined influences of the sympathetic nervous hyperactivity on urine production and micturition reflex in Dahl salt-sensitive rats fed with high salt diet. These rats revealed hypertension and increase in urine production during inactive periods of the day. Urine-producing rhythm collapsed when a heart failure model was made using an anticancer agent adriamycin with heart toxicity. An antidiuretic effect was clinically seen in short-acting anticholinergics administered before sleeping when sub-analyzed the data of clinical trial. On the other hand, an antidiuretic effect was not found in long-acting anticholinergics.

    We investigated the possibility of corticotropin releasing factor (CRF) as a causative factor for pathological bladder following chronic stress. Chronic stress was induced to rats by several methods; 1) repeated exposure to attack and defeat, 2) water avoidance stress, 3) communication box method in spontaneous hypertensive rat(SHR). No changes in urinary frequency were found in rats with repeated exposure to attack and defeat and with water avoidance stress. CRF protein level of the bladder was significantly decreased after exposure to water avoidance stress. mRNA expression of CRF and CRF receptor 1 of the bladder significantly increased in SHR with chronic stress when compared to that without stress. Adding CRF to the bladder muscle strips from the stress rat, carbachol significantly increased contraction dose-dependently. In conclusion, stress plus CRF may be involved in enhancement of the bladder contractions to acetylcholine in some pathological condition such as hypertension.

    We hypothesized that nocturia may be associated with the incidence of MetS and conducted a 4-year longitudinal study that investigated the possible association. We collected data on 5,701 individuals who participated in a multiphasic health screening in Fukui, Japan, in 2003 (baseline) and 2007, and were 65 years and less at baseline. After excluding participants with MetS at baseline, data from 5,234 participants were subjected to analysis. The participants' mean age was 55.6 years at baseline. A total of 210 participants (4%) developed MetS during the four-year study period. After adjusting for age and gender, a significant association was observed between the incidence of MetS and nocturia (experienced "sometimes" or "always"). For the incidence of MetS, the multivariate odds ratios (95% CI) of "sometimes" and "always" nocturia were 2.3 (1.1-4.7) and 2.9 (1.1-7.9), respectively. The results of our epidemiological study indicate that nocturia can be a precursor of MetS.

    We investigated whether the prevention of metabolic syndrome led to improvement of erectile dysfunction and lower urinary tract dysfunction. As animal models of metabolic syndrome, we used OLETF(Otsuka-Evans Tokushima Fatty) rats and Dahl salt-sensitive(DSs) rats, and investigated the function of the lower urinary tract. 1. The average body weight of male OLETF rats was significantly increased from the age of 24 weeks to 40 weeks, but significantly decreased from the age of 64 weeks. The systolic blood pressure was gradually increased from the age of 24 weeks. Plasma triglyceride, cholesterol, insulin and glucose were significantly higher in OLETF rats than in LETO rats both at the age of 24 and 56 weeks. These characteristics suggest that OLETF rat could be a model in evaluating the relationship between MetS and LUTS. 2. The urinary frequency for 24 h was significantly greater in OLETF rats from the age of 24 weeks, and the mean voided volume was significantly greater from the age of 56 weeks. This suggests that the male OLETF rats have micturition characteristics of frequent micturition and polyuria from early stage to late stage. 3. On cystometry, non-voiding contractions(NVCs) was observed in OLETF rats at the age of 24 weeks, and markedly increased at the age of 56 weeks. The bladder weight, maximum bladder capacity and residual urine were significantly increased at the age of 56 weeks. 4. The amount of ATP, PGE2 and NGF released from 20 cm H2O pressure-stretched bladder of OLETF rats was, respectively, significantly increased 3. 87±0. 46, 2. 80±0. 34, 1. 37±0. 22 times as much as those of LETO rats. The amount of 8-OHdG in the bladder of OLETF rats was 4. 71±0. 68 times higher than that of LETO rats. 5. The sympathetic nerve activity is augmented with salt loading in Dahl salt-sensitive rats(DSs), but not in Dahl salt-resistant rats(DRs). Six-week-old male DSs and DRs were fed with a normal(0. 3%) or high-(8%) salt diet and 40ml water/day for 12 weeks. Blood pressure was measured through the tail artery under non-anesthetizing state. Urine volume and frequency were recorded all day long in metabolic cage. We calculated inactive period(sleeping) urine volume index(IUi), which was defined as urine volume during inactive period divided by 24-h urine output. Twenty to 30 mmHg increase in systolic and diastolic blood pressure was noted in DSs after 2 week-salt loading. Salt loading increased urine production during active(awake) period of the day both in DSs and DRs. In inactive period, mean voided volume gradually increased in DRs fed with normal or high salt diet, while did not change in DSs fed with high-salt diet. There were significant differences in mean voided volume after 10 week-salt loading between DRs and DSs fed with high-salt diet. In DSs fed with high-salt diet, IUi increased 2 weeks after salt loading, and remained normal range within 4 weeks.

    We recently showed that Id2 mutant mice develop hydronephrosis with congenital obstruction at the ureteropelvic junction, the characteristics of which show a close resemblance to those of human congenital hydronephrosis. We investigated whether Id2 was involved in the pathogenesis of hydronephrosis using unilateral ureteral obstruction (UUO) mouse model. After 3 days of UUO, Id2 gene expressions of renal pelvis were increased by 2.4-fold. Ingenuity pathway Analysis and microarray expression data analysis from public data database suggested that Id2 gene related with Angiotensinogen.

    We investigated the relationship between polymorphism of metabolic syndrome-related gene and occurrence of lower urinary tract symptoms, using human blood samples and animal pathological models. Organ ischemia is thought to be caused by matrix digestion by metalloproteinases (MMPs) leading to rupture of atherosclerotic plaques. Production of macrophage MMP-2 and MMP-9 is induced by cycloxygenase-2 (COX-2) and prostaglandin E2 (PGE2) synthesis. The-765G->C polymorphism of the COX-2 gene has been reported to associate with a decrease risk of myocardial infarction and stroke. Detection of this genotype may be useful to predicting genetic risk of bladder storage dysfunction because bladder ischemia is known as one of the important risks for lower urinary tract symptoms (CUTS). Using blood samples of patients with overactive bladder (OAB), the-765G->Cvariant was genotyped. As the prevalence of the variant was low in these patients, the significant relationship was not found. Next, to investigate the association between the urinary levels of PGE2, PGF2α, nerve growth factor (NGF) and substance P, and OAB symptoms, urine samples from 114 patients in the chronic phase of a brain disease and 27 healthy controls were analyzed by enzyme-linked immunosorbent assay. As a result, the urinary PGE2 level was putatively elevated in patients with suprapontine brain diseases and associated with the presence of OAB. We collected data on 28,238 individuals (9,286 males and 18,952 females) who participated in a multiphasic health screening in Fukui, Japan. The overall prevalence of nocturia (defined as two or more voids/night) was 5.8%. In the multivariate analysis, a significant association was found between nocturia and the followings:age, male gender, BMI<18.5, BMI>27.0, high blood pressure and impaired glucose tolerance. The number of components of MetS was correlated with nocturia. Using rat animal models with cerebral infarction (CI), we investigated the underlying mechanism of increase in the urinary PGE2 level in patients with suprapontine brain diseases. The ATP amount in the bladder of CI rats was 2.24 times larger than that of sham operated(SO) rats 6 hours after CI. Compared with SO rats, the mRNA expression of P2X3, ASIC2, TRPV1, M2 and M3 in the bladder wall increased after CI, meaning upregulation of C fiber afferent nerves of the bladder. These changes were not found in rats pretreated with resiniferatoxin. As an animal model of metabolic syndrome, rats were fed with fructose-rich diet for 11 months. Rats revealed hypertension, glucose intolerance, and frequent voiding. The expression of COX-2 in the bladder wall was found to increase in these rats.

    It has been reported that a new variant in the COX-2 promoter, a guanine to cytosine substitution at -765 (-765G->C) is located within a putative binding site for the transcription factor Spl. The polymorphism of the COX-2 gene was found to associate with a decreased risk of myocardial infarction and stroke by reduction in PGE2 production. Previously we reported that PGE2 played an important role in neuron-plasticity in the brain and spine, which resulted in the development of OAB. Therefore, in BPH or stroke patients with -765GC genotype or -765CC genotype, development of OAR in believed to be diminished. In the present study, the patients with cerebrovascular accident, spinal lesion, or bladder outlet obstruction (BOO) were classified into two groups ; overactive bladder (OAB) group and non-overactive bladder (non-OAB) group according to the International Prostate symptom Score (IPSS)/OAB symptom score. Blood and urine samples were collected from all patients. Gemonic DNA was prepared from blood and -765G->C variant was genotyped by restriction endonuclease digestion of the polymerase chain reaction product. Genotypes were determined. The level of prostaglandin E2 (PGE2) in the urine were analyzed with the severity of OAB. The results obtained in 2006 and 2007 were as follows; 1. The urine levels of PGE2 in patients with BOO were significantly higher than that in control. 2. The urine level of PGE2 in patients with cerebrovascular accident was correlated with severity of IPSS. The patients with high score of IPSS reveals high level of urine PGE2. 3. COX inhibitors dose-dependently inhibited detrusor overactivity caused by cerebral infarction in rats. This effect was dependent on suppression of C-fiver afferent nerves in the bladder. 4. Single nucleotide polymorphism (SNP) in COX-2 promoter gene was found in 3 out of 70 patients with OAB. The results will be presented at 2008 NBS annual meeting in Tokyo. We have to accumulate the samples to analyze the correlation between SNP and severity of OAB. PGE2 excreted from the urothelium in response to bladder distension stimulates C-fiver afferent nerves, subsequently promoting detrusor overactivity. This mechanism seems to be true in patients with BOO. However, the levels of PGE2 were related with the severity of OAB in patients with cerebrovascular accidents. They were not proven to exist BOO. These results suggest that there might exist other underlying mechanisms to explain the role of PGE2 in OAB.

    Id2遺伝子欠損マウスが先天性水腎症を高頻度に発症することは、我々が独自の解析に基づいて明らかにし、世界に先駆けて報告したものである。その病態解析の結果からヒト水腎症症例におけるId2遺伝子異常の存在が強く示唆される。本研究は、遺伝子解析の手法を駆使して、ヒトにおける、Id2遺伝子の変異と先天性水腎症発生の因果関係を明らかにすることを目的とした。具体的には、ヒト先天性水腎症患者におけるId2遺伝子変異の有無を検討することと、既知の水腎症関連遺伝子であるBMP4に対するId2遺伝子発現の応答性の検討を行った。 (1)共同研究施設の患者の中で、先天性尿路奇形が家族内に認められる20家系(約100人)を対象とした。現在までのところサンプル採取できた7家系28サンプル(先天性水腎症13例、他の先天性尿路奇形2例、健常者13例)のゲノムDNAを解析した。3つのエクソン、2つのイントロンについて塩基配列を決定し、GenBankに登録されているヒトId2遺伝子座の塩基配列と比較したが、塩基配列を決定した28サンプルいずれにおいても変異を認めなかった。 (2)BMP4は分泌タンパク質であり、細胞膜表面の受容体を介してシグナルが細胞内に伝わる。この際、転写因子であるSmad1,Smad5,あるいはSmad8を介する。そこでId2遺伝子のプロモ-ター領域を用いたレポーターアッセイを行い、Smad1の存在下にId2遺伝子のプロモータ-が応答するかを検討した。しかしながらId2のプロモーターではSmad1に応答が見られなかった。この不応性は、検討するId2プロモ-ター領域の長さを変えても同様だった。 現在、未解析の症例が13家系残っており、今後も解析を継続する。また、プロモーター解析については、解析範囲の拡大とSmad1以外の補助因子による機能的強調の必要性の有無などについて今後検討していく。

    平成16年度にわれわれは、まず^<18>F-fluoroacetate(FAC)の合成方法を確立した。^<18>Fは当施設内にある小型サイクロトロンを用いて製造し、実際のFACの合成はo-mesyl-ethyl-glycolateをacetonitrileの存在下でフッ素化反応させて合成した。FACの組織内代謝に関する検討では培養細胞を用いた実験を行った。培養細胞には予備実験として、まず最初にLS174Tを150mm培養皿に準備し、培養皿に15mciのFACを加えて4時間培養した。培養液と細胞を分離し、それぞれの放射能を測定するとともに、細胞はホモゲナイズし、水溶性成分と脂溶性成分に分離した。それぞれの分画液を薄層クロマトグラフィー(TLC)法を用いて、分画液中の物質を同定し、それぞれの物質あたりの放射線量は放射線イメージ分析器を用いて測定することで、FACの集積量を決定した。その結果、11C-acetateでは脂溶性成分への取込みが多かったのに対して、FACでは水溶性成分への取込みが主であった。^<18>Fの脂溶性成分への取込み分画を調べると、主にphosphatidylethanolamineへの取り込みが多く、続いてphosphatidylserineとphosphatidylinositolの順に取り込みが多いことが分かった。平成17年度には、前立腺癌細胞を用いた細胞実験とともに、in vivoによる実験、すなわち前立腺癌移植マウスを用いた、組織内FAC分布を検討するとともに、移植マウスをPET撮像し、腫瘍組織が画像的に検出可能であるかについても検討する予定である。

    INTRODUCTION : Patients with complicated renal cysts present a diagnostic dilemma. Renal cancer with multiple renal cysts would be amenable to nephrectomy with curative intent, whereas cystic disease would be followed with watchful waiting. In this study, we evaluated the effectiveness of positron emission tomography (PET) with ^<11>C-acetate (AC) for evaluation of patients with complicated renal cysts. We also compared the reliability of AC-PET in detecting renal cancer with that of PET with ^<18>F-fluorodeoxyglucose (FDG). METHODS : Thirteen patients with complicated renal cysts were enrolled in this study. Following administration of 10 mCi of 11C-acetate, whole-body PET images were obtained. After allowing for 11C decay, 5 mCi of FDG was administered and repeat whole-body imaging was performed. The PET studies were scored as positive or negative in the kidney. PET findings were correlated with CT. RESULTS : Histological confirmation was confirmed in 11 patients. Six patients were diagnosed as renal cell carcinoma after surgery. Three of 6 patients with renal cell carcinoma (50%) had positive AC-PET findings, while only 1 FDG-PET study was positive (16%). Surgical specimens showed two patients had different type of tumour ; one patient had squamous cell carcinoma and the other had metastatic tumour. AC-PET showed positive finding for metastatic renal tumour, while FDG-PET was negative. None of these cases without malignant renal tumour had positive AC-PET or FDG-PET findings. CONCLUSION : AC-PET demonstrates marked uptake in renal cancer and higher sensitivity than FDG-PET. These preliminary data show that ^<11>C-acetate is a possible PET tracer for detection of renal tumour.

    Development of bladder overactivity (BO) caused by cerebral infarction (CI) is believed to require RNA transcription in the pontine micturition center (PMC). We previously reported that the expression of cyclooxygenase-2 (COX-2) mRNA was mediated by the activity of an NMDA (N-methyl-D-aspartate) receptor in the PMC and necessary for the development of BO. This study was undertaken to examine whether the expression of prostaglandin (PG) E or D synthase, downstream gene of COX-2, and levels of PGE2 and D were related to BO. To begin with, background mouse (C57-BL/6J) of COX-2 knockout was used to examine whether it has the same response to CI as a rat model. To confirm that the expression of COX-2 is essential for the development of BO, in vivo RNAi in the mice PMC and left middle cerebral artery (MCA) occlusion are thought to be necessary. Methods : Cerebral infarction (CI) was induced by MCA occlusion in female SD rats and C57-BL/6J mice. Awake animals were cystometrically examined. Specimens were obtained from the dorsal pontine tegmentum (DPT) 0.25, 1, 3, 5, 12, and 24 hours after MCA occlusion or a sham operation (SO). Expressions of PGES and PGDS in the DPT were monitored with real-time PCR. To determine whether PGE2 is essential for the induction of BO, microinjection of PGE2 into the PMC or periaquaeductal gray (PAG) was performed in urethane anesthetized rats. Furthermore, ONO-8711, EP1 receptor antagonist was administered intravenously or intracerebroventriculaly before PGE2 instillation. Results : Bladder capacity of CI rats and mice was significantly reduced 1-24 hours after MCA occlusion. One hour after MCA occlusion, PGES and PGDS mRNA expression had significantly increased, as compared to that in SO animals. PGES expressions remained consistently higher than those in SO rats at least 12 hours after MCA occlusion. Pretreatment with MK-801 inhibited the development of bladder overactivity and significantly reduced the expression of PGES mRNA in the DPT. The level of PGE2 increased 3 to 5 hours after MCA occlusion at the DPT. Microinjection of PGE2 into the cerebral ventricle and dorsal pontine tegmentum increased bladder capacity. PGE2 into the PAG significantly decreased bladder capacity, which was antagonized by the administration of ONO-8711. MCA occlusion produced a significant reduction in bladder capacity and increased expression of COX-2/PGES in the DPT of C57-BL/6J mice. Conclusion : These results indicate that the development of BO following MCA occlusion is mediated by an increase in COX-2 and PGES mRNA expression in the DPT. Increase in PGE2 level in the PAG is believed to play an important role in the development of BO caused by CI.
