    BACKGROUND: Patients cannot always share all necessary relevant information with doctors during medical consultations. Regardless, in order to ensure the best quality consultation and care, it is imperative that a doctor clearly understands each patient's agenda. The purpose of this study was to analyze the process of developing a shared-agenda during family physician consultations in Japan. METHODS: We interviewed 15 first time patients visiting the outpatient clinic of the Department of Family Medicine in the hospital chosen for the investigation, and the 8 family physicians who examined them. In total we observed 16 consultations. We analyzed both patients' and doctors' narratives using a modified grounded theory approach. RESULTS: For patients, we found four main factors that influenced the process of making a shared-agenda: past medical experiences, undisclosed but relevant information, relationship with the family physician, and the patient's own explanatory model. In addition, we found five factors that influenced the shared agenda making process for family physicians: understanding the patient's explanatory model, constructing the patient-doctor relationship, physical examination centered around the patient's explanatory model, discussion-styled explanation, and self-reflection on action. CONCLUSIONS: The findings suggest that patient satisfaction would be increased if family physicians are proactive in considering these factors with respect to both the patient's agenda, and their own.

    これまでに、医療面接のエキスパートによるノミナルグループディスカッションを2回実施した結果抽出された128の評価項目候補を元に、研究者間でディスカッションを重ね、評価項目に優先順位をつけるためのwebアンケートを作成した。webアンケートにまとめるにあたっては、MAAS-Global Rating List for Consultation Skill of Doctor;by JV Thiel, P Ram, JV Dalen Maastricht University, Netherlands） を参考に3部構成とした。 webアンケートは、第1部「医療面接の流れに沿った各段階のコミュニケーションスキル」について23項目、第2部は「一般的なコミュニケーションスキル」17項目で構成し、それぞれの項目に対して、9段階で重要度を尋ねることで研究者間で合意した。第3部はMedical Aspectsとし、今回のアンケートから外した。 日本プライマリ・ケア学会の協力を得て、専門部会のメーリングリストを使用し、アンケートを配布した。 上記の結果、重要であると判断された項目で作成されたものを暫定版評価表として、実際の専攻医の外来診療場面を録画（DVD）し、それを用いて暫定版評価表の機能評価を複数回行うラピッドプロトタイピングという手法で、ブラッシュアップし、最終版評価表を完成させることにした。 そこで、先ずは2施設2名の専攻医の外来診療を、専攻医、患者の双方の許可を得てビデオ録画することとし、実施した。今年度は、webアンケートをもとに暫定版評価表を作り、専攻医のビデオ録画をもとに機能評価を重ね、評価表を完成させ、併せて、マニュアル作成に取り組む予定をしている。

    今年度は、昨年度情報収集した地域における高齢者のニーズやコミュニケーションのあり方を元に、１年生を対象に、高齢者との援助的関係構築のための15コマのコミュニケーション教育プログラムを作成した。医療者になる前の早い段階で、患者を一人の生活者と捉え、「傾聴」「共感」「受容」の基本的コミュニケーションスキルを実践できるように強調した。5月下旬、学生同士のロールプレイの演習を経て、高齢者SP16名にご協力いただき、看護面接を実施し、６人１グループで、他の学生を観察しながら、コミュニケーションスキル評価をして、フィードバックを相互に行った。た。学生は緊張しながらも、繰り返し行うロールプレイとSPさんのフィードバックから多くの学びを得ていた。 本実習の教育効果を測定するための質的調査を実施するために、倫理審査の承認を得たが、本科目の責任者であったため成績判定を学生に報告する前に実施することは倫理的に認められず、結果的に９ヶ月後の結果を調査することとなった。ポスターにて学生の協力募集を行い、7名の学生がインタビューに参加してくれた。４人、3人の２グループを対象にエンタビューを実施した。その後、新型コロナウィルス感染拡大防止のため実施を見合わせている。 現在、インタビューの逐語録を作成し、質的分析を実施しているところである。

    退院前多職種連携カンファレンスで、実際に看護師がそのような役割を果たしているのかの現状を把握する目的で参加観察を行った。退院前多職種カンファレンスへの参加観察は、7ケース実施し現在分析中である。インタビュー調査に関しては、医師へのインタビューは終了した。看護師は、あと2～3例を予定している。コメディカルの多職種については現在日程の調整中である。 分析については、得られたデータから順次行う予定である。各データーを総合的に分析し関連等を検討していきたい。 学生たちにIPEの影響を明らかにするために、看護学生と薬学生との多職種カンファレンスを模擬事例を使用し行った。その前後の学生の変化を、RIPLS(The Readiness for Interprofessional Learning Scale),TEIQue-SF(Trait Emotional Intelligence)の2つの質問紙を使用して評価したが、RIPLSの下位項目であるチームワークとコラボレーションに薬学生に優位な変化が見られたのみであった。1度のみのIPEでは学生たちに大きな変化がないことが明らかになり、繰り返しの教育の必要性が示唆されたと考える。

    Simulated patients whose citizen receive certain amount of training and perform patient play an important role as an educational personnel for communication education. It is said that currently, more than 1000 SPs are actively working as an SP. However, as SPs increase, the lack of exercise and feedback exercises in medical interview education, and insufficient standardization of SP performance in OSCE have long been pointed out. In this study, we prepared evaluation questionnaire on SP performance (acting / feedback) and verified the validity. Also, using the evaluation questionnaire, we requested the performance investigation of the SP belonging to the SP groups nationwide. As a result, the performance evaluation of SP of 26 universities, 269 was obtained, the characteristics of the present SP's performance and feedback, the years of experience of SP, the practice time, the attributes of target students, and the relationship with the task were clarified.
