    本研究では、①医療事故の体験も可能な人工呼吸器複合現実シミュレータの開発と、②機械学習を用いた特徴量抽出による学習評価アルゴリズムの構築を目的としている。
    ①に関しては、広島国際大学の二宮教授ラボにおいて人工呼吸器の操作パネルと測定値グラフの表示が可能な人工呼吸器トレーニングアプリSimmarを開発いただき、関東圏の2大学において大学院生約40名を対象としたユーザテストとアンケート調査を実施した。その結果、卒後1年目から人工呼吸器に携わったと回答した者が30名(71.4%)、学部レベルでSIMMAN-ALS5000を使用した経験のある12名のうち、8名が満足したと回答し、4名が不満と回答した。大学院レベルでSIMMAN-ALS5000を使用した17名のうち、12名が満足したと回答した。まともに習ったことがないという自由記載も認められた。同時に、特別支援学校における人工呼吸器装着児への看護師のかかわりについて360度動画教材を開発し、アンケートを実施したところ、教材の難易度について、学部3年次の学習内容として適切と回答した者が約8割、学部4年次として適切と回答した者が同じく約8割であった。2023年度はこれらの結果と学会発表後のディスカッションを踏まえ、教材の難易度を設定する。
    ②に関しては、九州工業大学の井上教授と共に機械学習において抽出すべきタスクについて検討し、先行研究に基づき、手袋着用から手袋廃棄までの気管内吸引の一連の流れを1回の気管内吸引とし、吸引前の判断指標となる「肺音聴診」、吸引後の看護介入である「体位ドレナージ」を含めた行動認識のためのアノテーションを進めている。翌年度は、習熟度を判定する分類器の開発を進める。

    2021年度は、救急医療における延命治療の代理意思決定支援モデルを構築することを目指し、第1段階：救急医療における延命治療の代理意思決定支援モデルの構成要素の抽出のため、救急医療において延命治療の代理意思決定を行った家族の体験および代理意思決定支援とその効果に関する国内外の研究論文を収集しメタ統合の方法論を用いてメタ統合する予定であった。 研究の進捗は遅れており、現在、国内外の研究論文を収集し、国内論文の分析は終了しているが、海外論文の分析が未着手のため、メタ統合に至っていない。また、2021年度は、第2段階：全国救命救急センターを対象に延命治療の代理意思決定支援の実態調査の質問紙を作成する予定であったが、メタ統合の結果に基づき質問紙を作成するため、こちらも着手できていない。2021年度に実施予定の内容を2022年度に実施する予定である。

    アクションリサーチ（2サイクル目）：モデル施設（大学病院・公立総合病院・高齢者専門病院）で専門看護師が中心となり、多職種チーム構築をスタートした。スタート6～10か月後に各施設の専門看護師との進捗状況をディスカッションした。大学病院では、医師・管理栄養士・MSW・摂食嚥下認定看護師の協力を得てチーム構築していたが、理学療法士の協力が得られない状況であった。協力の得られない原因は上司の協力が得られないことであった。この原因への対策には困難感を感じていた。公立総合病院は、従来から存在する心臓リハビリテーションチームを活用する方法を用いていた。ただし、高齢者のハイリスク症例は大学病院に紹介されている現状から、介入できていない状況であった。高齢者専門病院では、全くチームビルディング活動ができていない状況であった。他職種への働きかけの行動がとれておらず、他職種との関係性が構築できていないことが推察された。ここから①チームビルディングを担うべき看護師のチームビルディング推進力の必要性、②理学療法士が直接関与できないときの患者への介入方法の検討という問題点が抽出された。 ②に対して、術前から高齢患者に対して、自宅で実施可能な「フレイル予防体操」というパンフレットを作成することにした。研究協力者の理学療法士に依頼し、心負荷をかけない加齢に伴う筋力低下予防運動を考案してもらい、研究協力者の医師に監修を依頼し、高齢者にもわかりやすいパンフレットの作成を行った。

    本研究の目的は、胃切除術を受けた胃がん患者が自らの健やかな食生活をマネジメントするために、ICT(Information and Communication Technology)を活用した自己管理支援ツールの作成と、そのツール使用による自己管理支援プログラムを開発し、その効果を検証することである。 本研究は2段階構成のプロジェクトとして遂行しており、第１段階として前研究の調査結果をもとに、自己管理支援ツール（モバイルアプリ）の作成を行う。申請者が平成26～28年に実施した前研究では術後半年間の胃切除術後患者の実態調査を行い、セルフケア能力が高いと術後後遺症状が軽い傾向であることや、就労の有無など生活スタイルによってもセルフケア能力や後遺症状に差があった。さらに詳細な分析を行った結果、退院直後が最も後遺症状が強くセルフケア能力が落ちること、その後も時期によって症状が異なること、セルフケア能力の維持と向上を目指した定期的なサポートが退院後も必要であることが明らかになった。これら術後の時期に応じた症状の変化や対応、日常生活や食生活に関する情報等を整理し、アプリ開発業者および研究者との専門家会議にて検討しながらモバイルアプリを作成した。試用してアプリの操作感やシステムに関する使用感および問題点を抽出、および改良し、研究対象者が使用するための準備が整った。 研究の第2段階は、作成したアプリを活用した場合の胃切除術後後遺症状、セルフケア能力、精神的状態、食関連行動等の実態と関連を調査し、アプリ活用効果を検証する。

    The study period was one year, from May 2018 to April 2019. The total number of beds in the four hospitals was 2,689, with 64,898 new admissions during the year. The total number of Community associated Pressure Injuries （CAPI） patients was 324, with 9 people admitted twice with PIs and 2 people admitted three times during the study period, for a total actual number of 312. Seventy-eight percent of the patients were hospitalized from home, but 41% returned home as their discharge destination, 40% moved their residence to an aged care facility or another hospital, and the remaining 20% died during their hospitalization. To describe the characteristics of CAPI patients, a cluster analysis of the 272 patients with the least missing information on factors that cause PIs was performed, and they could be divided into three distinctive clusters: Group 1: self-care group, Group 2: low ADL group, and Group 3: very high risk group.

    To provide safe and effective tracheal suctioning, prompt and multiangle decision and delicate catheter operation must be acquired. We developed a Projection Mapping simulator which shows biological reactions of an adult patient by 3D Computer Graphics to train above mentioned skills. Several scenarios which frequently clinically occur, e.g. the drop of the arterial oxygen saturation by the prolonged suctioning time, was created. Using such scenario, experienced nurse's and nursing students' eye movement and motion was inspected to quantify the learning curve to obtain judgment skill and delicate suctioning catheter control. Particular results were shown about the difference of eye movement and motion between the nursing student group and the nurse group.

    It is important to create opportunities to confront physical restraints. Basic education asks questions that may affect the physical and mental pain of the patient being suppressed and the pain of the family. In addition, think about the suppression that becomes nursing as possible through the experience suppressed as exercises and the experience of tying people. In practical training, it is important to verbalize visits and experiences and share them among students. The experience of releasing the patient's restraint and providing safety and security will be a basis for thinking about physical restraint as a nurse in the future.In continuing education, it is required for the team to actively communicate the necessity of physical restraint, cancellation, and thoughts of patients and families so that restraints become routine and nurses do not stop thinking.

    Objective: The present study aimed to develop models for the facilitation of critical patients’ recovery by examining component factors of nursing practice. Methods: Focus group interviews with CNS and PreCNS were conducted to collect data: case examples of nursing care provided to facilitate patients’ recovery. The interview results were documented verbatim, and component factors related to nursing care and its facilitation were extracted and interpreted. The descriptions were summarized and classified according to their similarities. Results: Data were collected from five groups. Descriptions of nursing care provided for recovery were classified into some categorie.Discussion: Advanced and sophisticated nursing care was provided in cooperation with critical patients to maximize their abilities to recover, while carefully avoiding the risk of exacerbation for patients with an unstable condition, based on clinical judgments supported by “thought and practice”.

    In order to develop an adherence scale of patients undergoing open heart surgery, literature review was conducted at Japan and abroad. Although there were many literature on adherence, there was no research using the concept of adherence at the perioperative period as well as the open heart surgery. For this reason, we conducted an interview survey on surgical risk prevention behaviors for patients undergoing open heart surgery. As a result, the five elements "recognition of disease and symptoms", "recognition of surgery and surgery risk", "support from healthcare persons and families", "risk prevention actions", and "performance of risk prevention actions" were clarified. Based on these five factors, we have prepared a measure scale of 30 items.

    The purpose of this study is creates “Modified the Nursing program for colorectal patient “to reduce low anterior resection syndrome for colorectal cancer. The most important is how we can practice for many patients, so that we suggest patient education using the brochure. It is based on cooperation with the doctor. The method is patient education by nurse at preoperation, postoperation after discharge from hospital outpatient. The brochure include [①low anterior resection surgery defecation disorders characteristic] [②changes in the body with 3 Tips for spend a good day], [③how to way of life and thinking, and how to deal with of dealing with] from clinging to the bowel how to frequent put in a chart by using the decision tree the bond if you are attending a toilet, can be addressed.

    Aim:The aim of study is to developmentofa DAUGS scale to assess postoperative dysfunction , using the social rehabilitation Mitsuno classification of a patient's postoperative dysfunction. Methods: A survey of patients who postsurgery and reconstruction was conducted using DAUGS-20, and sent to about120 patients by mail. Statistical analysis was performed with SPSS. Results: A total of about 120 questionnaires were returned 20(31%).The mean age was 65.5±10.0 years(Mean±SD),including 22 men and 15 women. The DAUGS score is 29.5±19.2points(range20.7-68,with possible scores 0 to 100).A total of 17 patients (41%) returned to the same job as preoperatively, with a mean DAUGS score of 29.5±13.8.There were 20 patients(59%) who worked less than preoperatively with DAUGS score higher. Conclusions: The DAUGS scores correlated with the Mitsuno classification of dysfunction.The reliability and validity of the DAUGS20 scale for English version could be confirmed from socialcomeback situation.

    Aim:The incidence of gastric cancer in Japan is higher than in many other countries.Also,due to the advancement of early examination and diagnosis, operative treatment is primarly provided. However, patients, who have undergone reconstruction of the digestive tract live with postoperative functional disoders. In this study,we conducted a survey to standard value establishment of a gastric cancer patients postoperative functinal disorder in Japan.Methods:The cooperation of patient members of the alpha culb was obtained and a mail survy was conducted, involving 3,144 pepole.Result:Questionnaires were returned by 2,420 people(76.9%),with valid respnses from 2,343(96.8%).The mean postoperative weight loss was 9.4SD6.3kg.Patients average score under the dysfunction after upper gastrointestinal surgery)DAUGS scring system was 34.5SD14.7 points out of 100(range of 0 to 95 ).

    上部消化管がん患者の術後機能障害を評価するために、先行研究において術後機能障害評価尺度32項目(DAGUS-32 : Dysfunction After Upper Gastrointestinal Surgery-32)を確立した。しかし,患者の立場からより項目数が少なく,感度の高い尺度に洗練する必要があると考え,本研究に取り組んだ。3施設の共同研究で其々の施設の倫理委員会の承認を得た。対象は上部消化管がんの手術を施行し選定条件を満たした患者約1000名で,質問紙調査(DAGUS-20)を行った。回収数は992名,胃癌669名,食道癌222名,平均年齢65.7±9.8歳,食事にかける平均時間26.7±23.3分,体重減少-7.3±8.1kgであった。尺度の合計得点平均は,100点満点中31.3点(最小1-最大76)であった。尺度の有用性を検討するために信頼性と妥当性を検討した。信頼性の検討(クロンバックα係数20項目全体のCronbach'sα係数0.911,下位尺度のCronbach'sα係数0.612～0.856,再テスト法;合計得点の信頼性係数0.865),妥当性の検討(既知グループ技法,因子分析による下位項目の因子構造から因子的妥当性)を行った。探索的因子分析の結果,DAUGS32における術後機能障害は,20項目7因子から構成されていた。それらの因子名は,順に「逆流障害」,「活動力低下障害」,「食直後通過障害」,「ダンピング様障害」,「移送障害」,「低血糖障害」,「便通障害」に分類され,DAGUS-32同様の因子が温存された。今回開発したDAUGS20は,信頼性・妥当性ともに確保され,臨床への有用性の高い尺度である。

    研究目的:(1)上部消化管がん(胃がんや食道がん)切除患者の術後の栄養状態回復と食生活行動について,実態を把握する(2)術後の生活上の問題とその支援の方向性について検討する. 研究方法:選定基準を満たす上部消化管がん切除患者283名を対象に,自記式アンケート調査を配布・回収共に郵送法で行った.倫理的配慮として,自由意志による研究参加への同意,プライバシーの保護について,研究協力依頼書にて説明し,回答の返信をもって同意を得たとみなした.(2)術後の生活上の問題については,自由記述を依頼し、意味内容の類似性があるものをまとめ、その内容を忠実に反映させ、簡潔に表現した. 結果:胃がん190名(80.1%;男性132名,女性58名),食道がん47名(19.8%;男性44名,女性3名)であった.手術後の経過期間別では,3ヶ月以上6ヶ月未満27名,6ヶ月以上1年未満46名,1年以上2年未満70名:58.0±16.5点,2年以上3年経過94名:58.9±14.8点,であった.手術前後の体重減少の平均は,7.83±6.9kgであった.BMIの平均値は16.9±6.9であった.食事状況に関する質問内容の結果は,食事回数平均は3.01±0.39回/日(n=237),間食回数平均は1.76±0.93回/日(n=211),食事にかける時間は24.3±11.5分(n=237),食後の休息は41。0±30.0分(n=233)であった.他,自由記載の内容を質的分析した結果,術後の生活において,食事,移動,排泄,睡眠,コミュニケーション等の日常生活全般で困っている問題が生じていた.問題の中でも,特に,体力低下で困っているあり様が示されていた.今後は,食生活行動、ならびに体力低下に対する生活支援の方向性について具体的検討が必要であることが明らかになった.
