    1.研究方法の概要:20名から研究参加に承諾が得られ面接調査を実施した。 2.個人の背景:年齢は30~70歳、性別適合手術(以下SRS)実施は3名、戸籍の性別変更終了者は2名であった。就業状況は2名が無職、18名は有職であった。婚姻状況は、既婚4名、独身12名であった。 3.成人後の職場での困難や問題、化粧や女性の装いの意味:職場では【退職を強要】【職場でのセクハラやパワハラ】等の問題があった。女子更衣室を使えないことに対して【女性にはなれない現実】をつきつけられた。一方、女性の服装や化粧をすることは女性としてのアイデンティティを確認し【情緒の安定と女性としての自分を取り戻す】必須の時間と空間であった。美容院やデパートの化粧品売り場での【女性として普通に対応し受容してくれる】者はMtF当時者の重要な支援者であった。また【女性の人生を送りたい私を受容してくれる妻】は心の支えであった。 4.治療状況、SRSに至った理由、SRSへの思い:ホルモン剤の開始を含め治療開始やSRS実施の動機は「女性で通用しない現実に直面」「子どもの成人」「中年男性の体臭に嫌悪」等であった。SRS未実施の理由は、「子どもの教育費の確保」「親の性別変更は子供の不利益」「SRSは体力的に心配」「ホルモン治療で外見が女性化し満足」等であった。一方で【本物の女性でないぶん努力しなければ女性に追いつけない】【全身治療し完全な女性になりたい】との思いもあった。また、SRS実施者は「治療は私の救い、治療により本当の人生が始まる」「治療したことで本当の自分を取り戻した」と【女性性の回復】【治療したことへの満足】を述べていた。 5.性別違和(性同一性障害)の意味付け:「治療で多くのものを無くしたが、得た物のほうが多い。この病に生まれついたのは尊く何かの使命」と、【苦悩を乗り越え性別を超越した人格的発達】に至った当事者もあった。

    In 2014,the JMA issued new guidelines according to which midwives working at maternity homes were allowed to handle the deliveries for GBS-positive pregnant women only if they comply with the guidelines of the JSOG and promote cooperation with commissioned obstetricians and paediatricians. However, whether the midwives actually comply with these guidelines is not known. I investigated compliance or the opinion to the guidelines about the GBS targeting at a midwife, an obstetrician and pediatrician. Though about 66.2% dealt with the delivery of the GBS-positive pregnant woman as for the midwives at maternity homes, the compliance rate was low, and it became clear that a rate well-known to not only the existence of pediatrician and obstetricians but also the revision was low. For improving compliance rates, midwives at maternity homes should discuss the management of GBS-positive pregnant women with commissioned doctors more carefully and concretely per the existing guidelines.

    This project explored the feasibility of developing and introducing online education on systematic review for midwifery students. We first conducted a needs assessment among midwifery and nursing students regarding the knowledge, attitude, and practice of systematic review, which indicated the necessity of providing education to meet students’ needs. We then visited three universities in the UK to learn the current practice on systematic review education there. Based on what we learnt from the UK, we conducted a series of seminars on systematic review for midwifery students. Based on the feedback from the students, we found face-to-face interactive learning to be more advantageous than online education, especially for the early-stage learners who often encounter difficulties in learning the process of systematic review, therefore, need additional consultation.

    This project aimed to develop and evaluate the effectiveness of an educational intervention for pregnant women regarding caesarean birth. The design and construction of the educational material was based on a series of studies, including a qualitative study on women who had traumatic caesarean birth experience and a needs assessment on antenatal education regarding caesarean sections for pregnant women as well as consultation with service users, health-care professionals (obstetricians and midwives), and information communication specialists. Following a pretest, we conducted an RCT in one of the university hospitals to evaluate the effectiveness of the educational intervention to improve women’s psychological health (e.g., improving sense of control during labour and birth, and reducing posttraumatic stress symptoms and depressive symptoms following childbirth). The study is in its final ongoing stage so as to provide further information about the effectiveness of the intervention.

    There are little study for striae gravidarum in foreign coutries as well as in Japan. The time of appearance for striae gravidarum is also not been clarified at present. I studied the relationship between the presence of accurence for striae gravidarum and serum relaxin level. There was a case which striae grabidarum had appeared at 28 weeks of gestation, but many had appeared 34 weeks gestation. There was no significant difference between the presence of accurence for striae gravidarum and serum relaxin level. I could not see the process of accurence for striae gravidarum and serum relaxin level.Therefore further research will be required to explore the cause of striae gravidarum.

    Although gestational diabetes mellitus (GDM) is one of the evidential perinatal complications, this disorder of carbohydrate metabolism in gestational period impacts on not only women during pregnancy, but also them after the delivery. It is well-known that exercise habits in daily life are effective to improve carbohydrate metabolism in DM patients, however, no evidence can be found whether the physical activity may normalize glucose level among pregnant women. To explore the relationship between life activity and carbohydrate metabolism among Japanese healthy pregnant women in the second trimester, we have alredy analyzed the blood sugar levels between two groups divided according to waking frequency. The results indicated that life activity as walking was effective for carbohydrate metabolism even for pregnant women. AS the next step, we have performed walking intervention to women with GDM, and serum casual glucose and hemoglobin A1c levels were measured.

    We developed a chewing gum containing folate, whose effects for folate supply and emesis improvement were analyzed. A cross-sectional study on pregnant women in early pregnancy who had presented emesis was conducted. A gum containing folate or an identical gum without folate was given to women at early gestation for two weeks. Emesis signs were estimated using a emesis scoring system. As results, the chewing gum containing folate would be effective for emesis improvement for early gestational women.

    The study about striae gravidarum is not reported including foreign countries as well as Japan, and it is not found about appearance mechanism and the cause of striae gravidarum. I clarified it with the QOL evaluation questionnaire about the influence for the pregnant women that appearance and preventive steps of striae gravidarum for the first time. Among pregnant women who took preventive steps, there was no significant difference in scores for QOL between the presence and absence of striae gravidarum. Therefor I conclude that preventive steps for striae gravidarum should be reconsidered from the QOL viewpoint for pregnant women.

    This study was conducted to examine the current status, awareness and education of ultrasonic diagnosis performed by midwives. 1) The numbers of midwives and obstetrician were significantly lower in facilities where ultrasonic diagnosis was performed by midwives. 2) There were none differences between doctors and midwives with regard to the psychological effects of the examination on pregnant women or the diagnostic skills. 3) 0n the education of the ultrasonic diagnosis for midwifery students, About 10 times practices were necessary at least, until the accurate measurements of fetus were possible. 4) The pregnant a fetus in 20 weeks) for midwifery education using ultrasound scan was developed. Based on these results, prenatal checkups using ultrasonography examinations performed by midwives should become more widespread. It is important to develop aneducation system and consultation system in an abnormal case.

    本研究の目的は、不妊治療後妊娠者とそのパートナーの周産期(妊娠期・分娩期・産褥期)の身体的・心理的・社会的問題を明らかにし、安心して周産期をすごせるためのケアガイドラインを構築することであり、本年度が最終年である。 1. 平成19年~20年に亘って行った調査のまとめ 平成19年~20年に不妊治療を実施している施設で働いている看護職者に対する調査結果をまとめて、女性心身医学に投稿した。平成20年度の後半に実施した不妊治療センターを併設する京都府内の産婦人科1施設における未経産の妊婦147名に対する調査は、3つの学会で報告した。 2. ニュージーランドにおける不妊治療後妊娠者のケアガイドラインを調査 ニュージーランドの病院および助産院において、助産師教育および助産師業務の中の不妊治療者および不妊治療後妊娠者のケアを視察した。ニュージーランドの助産師教育の中では、ハイリスク妊婦の分類の中で不妊治療者および不妊治療後妊婦へのケアが教育されている。しかし、実際のケアにおいては、特に不妊治療者および不妊治療後妊婦に対する特別なガイドラインは創設されておらず、わが国と同様である。わが国でも、本調査で看護職者は不妊治療後妊娠群に対して特別のニーズや特別なケアの必要性を高く認識していること、および,不妊治療後妊娠群は自らを特別だ,特別に取り扱って欲しいと捉えているにもかかわらず,自然妊娠群と岡様に扱っているという実態が認められ,認識と実践の不一致も明らかとなった。また、ケアで重要な時期として,「妊娠初期£と「産褥期」を挙げ,不妊治療後妊娠群に対して精神的なケアやカウンセリングの必要性を示唆された。今後は看護職に対して不妊治療後妊娠者に対する教育の機会を増やすだけでなく,不妊治療後であることを踏まえた周塵期の時期別ケアに関する教育など教育内容の充実を図ることの重要性が示唆された。
