    1.超短時間型CBT-Iの開発(2023年10月迄):先行研究で開発した短時間で実施可能な不眠に対する認知行動療法(Cognitive Behavioral Therapy for Insomnia;CBT-I)を、面接回数を減らしてさらに所要時間を短縮するよう改良した。併せて睡眠アセスメントシート・教育用シート、睡眠記録シートも改良、刷新した。その上で、セラピスト向けの実践マニュアルを作成した。 2.セラピストのトレーニング(同年12月迄):全国4か所の病院・クリニックに所属するセラピスト(コメディカルスタッフ)を対象に、定期的な実践トレーニングを遠隔で実施した。共同研究者らが医療コミュニケーションの基本、CBTの基本、本研究で開発した超短時間型CBT-Iの研修を行った後、対面でのワークショップを開いて実践的な演習を行った。 3.研究倫理審査ならびに研究協力施設の実施許可申請(同年12月迄):富山大学臨床・疫学研究等に関する倫理審査委員会に倫理審査を申請し、承認を得た(R2023152)。その後、研究協力施設より、実施許可を得た。 4.臨床試験登録(2024年1月):本研究について、開発した超短時間型CBT-Iの有効性を検証する多施設共同無作為化並行群間比較試験として臨床試験登録を行った(UMIN000052911)。 5.臨床登録・介入・スーパーバイズ開始(2024年1月末~):研究協力施設との連絡・調整、インフォームドコンセントの準備、臨床登録・データ入力のシステム配備などを経て、臨床登録を開始した。登録者数はまだ少ないが、介入群に実施される超短時間型CBT-Iの録音データをもとに、各セラピストを担当する共同研究者が継続的にスーパーバイズを実施している。

    研究開始当初、勤労者向けの「レジトレ!version 2.0」はすでに完成していたが、それを参加者モニターに実際に体験してもらって改善すべき点に関する意見をまとめ、アプリ制作会社であるライフツービッツ社との週1回のミーティングを繰り返しながら、「レジトレ!version 2.1」を完成させた。また、研究のホームページを作成し、研究参加者がそこからエントリーできるシステムを構築した。 また、JMDCと月1回のミーティングをしながら、JMDCが提携している各種健康保険組合と登録企業に対して、本研究参加の声かけを行い、申込者に対するオンライン説明会(最大40名参加)を1日3回実施し、目標エントリー数に達成した。参加者に対して毎週、励ましメールを送信し、モチベーションの維持に努めた。その結果、割り付けられたスキルによって完遂割合は異なるものの、約70%から95%の参加者が最終レッスンまで終えることができた。そのうち、2ヶ月目のアウトカムを測定できた参加者の割合は約97%である。 まだ割り付けをオープンにしていないため、あくまですべてのスキルを合わせた全体の結果ではあるが、抑うつ症状PHQ-9の得点はアプリ開始前と比べて約3点減少しており、このレジトレアプリの抑うつ症状改善効果は期待できることがわかった。 まもなく、主要アウトカムの「② 2ヶ月後の抑うつ症状」の測定が完了するため、その後の解析、すなわち、各スキルによって症嬢改善に差があるかについて準備を進めている状況である。

    本研究は、精神疾患を有する患者がプライマリ・ケア医療において心理療法に繋がることの出来る体制を構築する基盤整備のために、①プライマリ・ケア医療機関における心理療法の提供に関する実態を明らかにし、②実装戦略を確立することを目的としたものである。この目的の達成のため、当該年度は以下を実施した。1)昨年度から継続し、ステークホルダー(プライマリ・ケアの専門家、心理専門職、臨床心理学・精神医学の専門家、実装科学の専門家等)との定期的な意見交換、2)日本のプライマリ・ケア領域における心理療法の有効性検証に関する先行研究のレビュー、3)CFIRに基づく調査を行うためのツールの開発や整備、実施者のトレーニング、パイロットインタビューの実施。加えて、プライマリ・ケア医療機関における心理療法の実装基盤の構築や行動科学領域における普及と実装科学の普及を行なっていくために、行動科学領域における普及と実装科学コミュニティ(Dissemination and Implementation Research Community: DAICOM)の活動を継続し、現在、多様なバックグラウンドの参加者約200名と共に勉強会や普及と実装科学に基づく研究実施のトレーニングを行っている。

    本研究は、不眠症患者に対するテレビ会議システムを活用した遠隔心理療法(認知行動療法)の有効性を検証するとともに、その作用機序及びわが国での心理療法の社会実装に資するデータを取得することを目的としたものである。この目的の達成のために、研究の第一段階として、不眠症患者を対象とした遠隔心理療法のプログラム開発とその有効性、安全性及びセンシング機器によって得られたマルチモーダル情報を活用した遠隔心理療法の作用機序を探索的に検証する臨床試験を実施する。 臨床試験を行うにあたり、昨年度までにテレビ会議システムとビデオ映像、紙媒体の介入マテリアルを組み合わせた介入プログラムの作成、データ管理システムの構築を行い、所属機関の倫理委員会に研究倫理申請を行った。また取得した要配慮個人情報の授受について頑健性が担保されたデータ授受システムの構築を行った。 本年度は社会実装を見据え、作成した介入プログラムや連携体制が医療機関において実施可能かヒアリングを行ったのち、それらの意見を反映させ、二つの臨床試験(CBTec study, Recipe study)を開始した。CBTec studyにおいては本年度内に目標症例数の研究組み入れを達成した。両試験とも研究対象者の研究組み入れと並行し、主要解析および二次解析に向けたデータ整理、すでにデータが得られたものについて学会発表や論文執筆等を行った。さらに、不眠症患者に対する心理療法を実施するセラピストの養成のためにオンデマンド研修およびライブ研修を実施した。

    Depression among university students significantly impacts academic performance and overall student life. In this study, we investigated the optimal components of smartphone cognitive behavioral therapy (iCBT) for preventing depression among university students. We randomly assigned 1,627 healthy university students to presence or absence of five components of iCBT. After an 8-week intervention, we followed them for one year. Acute analysis of 1,093 participants revealed that depression symptoms improved in all groups by the 8th week. A similar trend was observed in the total 1,627 participants at the one-year follow-up. However, no significant differences were found based on the presence or absence of specific CBT elements in terms of depressive symptoms or major depressive episodes. Despite this, our results provide valuable insights for designing clinical trials aimed at identifying effective iCBT components.
