  • M大学生の看護臨地実習における認識の特徴 日本語版実践力上昇尺度を用いた他大学生との比較, 曽我部 美恵子, 大原 良子, 安東 由佳子, 関西看護医療大学紀要, 10, (1) 35 - 47, 03 , Refereed, M大学の看護学生の看護臨地実習における認識の特徴を他学の大学生と比較し明らかにすることを目的に量的記述研究を行った。編入生・助産専攻学生を除く看護大学生を対象に、「日本語版実践力上昇尺度」を用いて2015年6月〜2016年3月に自記式質問紙でデータを収集した。M大学を含む5校の大学から協力が得られ、有効回答343名のデータを使用した。M大学生と他大学生の比較では、10項目に有意差を認め「実習指導者は、ずっと同じ人だった」「患者へのケアが学内で学んだものと違うときは、臆することなくそれについての意見を述べられた」「指導者は日々の実習終了時に、私の行った援助に感謝してくれた」「実習によって、看護師になる自信がついた」において、有意にM大学生が高いという結果であった。また、M大学生間の性差で有意差を認める項目は存在しなかった。看護師課程と保健師課程の比較では、「実習によって看護師になる自信がついた」は保健師課程の学生が有意に高く、「実習中、不安を感じた」「実習は苦しい体験だとわかった」など否定的な内容を問う5項目で看護師課程の学生が有意に高かった。(著者抄録)
  • 看護学生における臨地実習への取り組みに対する積極性に関連する要因の検討, 馬場 美幸, 安東 由佳子, 日本看護福祉学会誌, 23, (2) 79 - 91, 03 , Refereed, 本研究の目的は、看護学生における臨地実習への取り組みに対する積極性の関連要因を検討することである。看護学部3年生の学生計163名を対象に、無記名の質問紙調査を実施した。実習への積極性7項目を選定し、自己効力感、自尊感情、Locus of Controlとの関連を検討した。有効回答数は117部(回答率71.8%)であった。その結果、患者との関係形成に積極的に取り組んだ学生は、自己効力感と自尊感情とLOCが高かった。また、看護援助の実践、疾患・技術の自己学習、助言を求めたり相談すること、体調を整えることにおいて、積極的に取り組める学生はLOCが高かった。以上より、患者との関係形成に積極的に取り組むためには、自尊感情と自己効力感とLOCを高めること、看護援助の実践、自己学習、助言や相談の実施、体調管理の積極性を高めるためには、LOCを高めることが重要であることが示唆された。(著者抄録)

  • Development of a Burnout Causal Model for Nurses Caring for Patients with Intractable Neurological Illness, ANDO Yukako, KATAOKA Tsuyoshi, KOBAYASHI Toshio, OKAMURA Hitoshi, KITAOKA Kazuyo, Journal of Japan Academy of Nursing Science, 29, (4) 3 - 12, 12 21 , Refereed, Purpose: The purpose of the present study was to develop a Burnout Causal Model, to verify the fitness of this model and to provide suggestions for improving work-related environments for nurses caring for patients with intractable neurological illness.
    Methods: Subjects comprised 385 nurses working in the intractable neurological illness ward. The study was conducted by self-administered questionnaire sent by mail. A Burnout Causal Model was examined via Structural Equation Modeling.
    Results and Conclusion: There were 248 valid responses. Job stressors influencing burnout were “Quantitative workload” and “Conflict with superiors.” Among nurses with less than 3 years of experience nursing patients with intractable neurological illness, “Verbal abuse” influenced burnout directly, whereas “Difficulty of involvement” influenced burnout indirectly. “Uncertainties regarding prospects of care” indirectly influenced burnout in nurses with 3 or more years of experience nursing patients with intractable neurological illness. Burnout led to turnover/reshuffle intention. “Conflict with colleagues” directly influenced turnover/reshuffle intention in nurses with less than 3 years of experience, compared to “Quantitative workload” and “Verbal abuse” for nurses with 3 or more years of experience. Among nurses with less than 3 years of experience nursing patients with intractable neurological illness, turnover/reshuffle intention is strengthened if workplace supports are weak. Supports in the workplace are thus important for nurses with little experience. These findings indicate that measures need to be based on years of experience nursing patients with intractable neurological illness.
  • Development of a job stressor scale for nurses caring for patients with intractable neurological illness: selection of question items, 安東由佳子, 片岡健, 小林敏生, 岡村仁, 田中克俊, Journal of health sciences, Hiroshima University, 8, (1) 1 - 6, 12 , Refereed, The purpose of this research was to select question items for the development of a job stressor scale for nurses caring for patients with intractable neurological illness. The study was conducted by mail and used a self-administered questionnaire comprising 69 items, as clarified in previous research. The study period was from October to November 2005. A total of 221 questionnaires were distributed and 143 valid responses were received (valid response rate, 64.7%). Distribution of scores was confirmed via mean ± standard deviation. Differences in items were examined by item-total correlation, while similarity of items was examined by correlation of each item. Regarding content validity and face validity, questions were modified with reference to subject answers. The number of items in each factor was examined by factor analysis and Cronbach's alpha. As a result, 42 items were eliminated because these items were not fit for a job stressor scale for nurses caring for patients with neurological intractable illness. A scale comprising 27 items was then created.

  • Finding Out the Work-Related Environmental Stressor Influencing Burnout Among Nurses Caring for Patients with Neurological Intractable Illness, Ando Yukako, Kataoka Tsuyoshi, Kobayashi Toshio, Journal of Japan Society of Nursing Research, 29, (1) 45 - 55, 01 , Refereed, The purpose of this study is to clarify the work-related environmental stressor influencing Burnout by utilizing a Covariance Structure Analysis in order to plan concrete measures for preventing Burnout. The subjects were nurses caring for patients with neurological intractable illness. Questionnaire investigation was carried out to 284 nurses working in ward for these patients of national and general hospitals with 300 beds or more.
    Findings were as follows, the first work-related environmental stressor influencing Burnout is the factor of "impossibility on nursing". The second, that of "conflict with colleagues". The factors of "distrustfulness of doctors", "conflict with bosses" and "experience of patients' death" did not significantly influence Burnout. These findings indicated the importance of the work-related environment in which nurses are easy to express their feelings during patients's care and to establish comfortable relations among colleagues

  • パーキンソン病患者における抑うつ症状の発症・進行に関与する心理・社会的要因の検討, 安東 由佳子, 阿部 朱美, 奥田 鈴美, 植木 美乃, 松川 則之, 山脇 健盛, 小林 敏生, メンタルヘルス岡本記念財団研究助成報告集, (28) 7 - 11,   2017 03 , ベースライン調査(2014年)、1年後調査(2015年)共に回答が得られたパーキンソン病(PD)患者91名を対象とした。抑うつに影響する心理・社会的要因として、配偶者の有無、就業の有無、SOC(sense of coherence)を設定した。日本語に翻訳した13項目5件法の短縮版SOCスケールを使用した。全体の抑うつ得点をベースライン時と1年後で比較し、有意差は認めなかったが、ベースライン時の抑うつ有無別に、抑うつなし群および抑うつあり群で解析し、1年後の抑うつに有意差を認め、抑うつなし群は得点が上昇し、抑うつあり群は得点が低下した。1年後の抑うつ発症は43名中14名で、発症率は32.6%であった。ベースライン時に抑うつがなかった43名を対象として、心理・社会的要因の違いによって、1年後の抑うつ発症率に差があるか解析した。全ての要因において、有意差は認めず、身体的要因も関連がなかった。ベースライン時のSOCの違いによって、有意差があったのは、1年後の抑うつであった。

Research Grants & Projects

  • Investigation for inducting about the long period clinical placement at the same place which is adapting the Maslow's hierarchy of needs and construction of its training methods, Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Challenging Exploratory Research,   2015 04  - 2019 03 , This research was conducted to know the self-valuation of students about competence as nurse or midwife after the clinical placement. Data was gathered from 255 nursing students and 84 midwifery students by using the five-point Likert scale. Although, midwifery students experience longer clinical placement than nursing students at the same medical institution, nursing student are significantly higher self competence point than midwifery students at the below three items (p<.05):“As a result of the placement I am more committed to patient-centered care”; “During the placement I was able to improve my time management skills”; and “I am more able to integrate theory and practice as a result of the placement.”It was unable to identify by the self-valuation that the length of same place of clinical placement would improve competence of students' nursing or midwifery skill.
  • Concideration of an elderly support model using the Laughter Care Exercises Program in undrpopulated areas, Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Challenging Exploratory Research,   2015 04  - 2018 03 , The present study was to train supporters for the elderly and to investigate an elderly support model using this program in underpopulated areas. We surveyed health conditions of the elderly in such areas and investigated Laughter Care Exercise that are incorporated in this program and preventive care activity. The results suggest that individuals who live with laughter every day have better health view and higher physical fitness, suggesting the support for making a habit of laughter. The blood pressure was improved in subjects who did Laughter Care Exercise under the instruction of qualified local residents who participate in a preventive care activity as supporters for the elderly.
  • The effectiveness of "Interactive Experience Education for career development" program in nursing college students, Japan Society for the Promotion of Science, Grants-in-Aid for Scientific Research Grant-in-Aid for Challenging Exploratory Research,   2014 04  - 2017 03 , Along with the expansion of the health and medical field, nursing occupation has been increasingly expanding. In order to ensure smooth career development for nurses, profession focused career ducation is necessary from the early student life. In this research, "Interactive Experience Education for career development" was conducted among nursing college students, and its effectiveness was examined. As a result, it was shown that students with high motivation recognized improved career readiness after the career development education. Moreover, students’ strategic career planning ability was also improved. It is suggested that the effective implementation of "Interactive Experience Education for career development" will be attained after improving the motivation of nursing students.
