Endocrine-disrupting chemicals from the environment are considered to be involved in hypospadias onset through epigenetic alterations. This pilot study aimed to explore disease-specific methylated CpGs in human patient samples using the methylated-site display-amplified fragment length polymorphism (MSD-AFLP) technique developed by our research group [1]. We compared clinical samples from hypospadias and phimosis patients. Foreskin and blood samples were collected from one- to two-year-old patients with hypospadias (N = 3) and phimosis (N = 3) during surgical treatment. MSD-AFLP analysis showed significantly decreased CpG-methylation levels of genes such as MYH11 and increased CpG-methylation levels of genes such as PLA2G15 in hypospadias patients. Hierarchical clustering analysis showed that genes with significantly altered CpG levels were more markedly altered in DNA from blood than from foreskin. Because of the small number of samples, further investigation is necessary to elucidate the association between variations in CpG levels in foreskin and blood DNA and male genital abnormalities. However, our MSD-AFLP method appears to be a useful tool for exploring disease-specific methylated-CpGs in human epidemiological studies.

  • Involvement of the bone morphogenic protein/SMAD signaling pathway in the etiology of congenital anomalies of the kidney and urinary tract accompanied by cryptorchidism, Kentaro Mizuno, Akihiro Nakane, Hidenori Nishio, Yoshinobu Moritoki, Hideyuki Kamisawa, Satoshi Kurokawa, Taiki Kato, Ryosuke Ando, Tetsuji Maruyama, Takahiro Yasui, Yutaro Hayashi, BMC UROLOGY, 17, (1) ,   2017年12月, 査読有り, Background: Congenital anomalies of the kidney and urinary tract (CAKUT), such as renal dysplasia, hydronephrosis, or vesicoureteral reflux, are the most common causes of end-stage renal disease. However, the genetic etiology of CAKUT remains unclear. In this study, we performed whole exome sequencing (WES) to elucidate the genetic etiology of symptomatic CAKUT and CAKUT accompanied by cryptorchidism. Methods: Three patients with unilateral renal dysplasia accompanied by ipsilateral cryptorchidism were included in this analysis. Genomic DNA was extracted from peripheral blood, and WES was performed. Disease-specific single nucleotide polymorphisms (SNPs) were determined by comparison with the human genome reference sequence (hg19). Additionally, we searched for SNPs that were common to all three patients, with a particular focus on the coding regions of the target genes. Results: In total, 8710 SNPs were detected. Of the genes harboring these SNPs, 32 associated with renal or testicular development were selected for further analyses. Of these, eight genes (i.e., SMAD4, ITGA8, GRIP1, FREM1, FREM2, TNXB, BMP8B, and SALL1) carried a single amino acid substitution that was common to all three patients. In particular, SNPs in SMAD4 (His290Pro and His291Pro) have not been reported previously in patients with symptomatic CAKUT. Of the candidate genes, four genes (i.e., ITGA8, GRIP1, FREM1, and FREM2) were Fraser syndrome-related genes, encoding proteins that functionally converged on the glial cell-derived neurotrophic factor/RET/bone morphogenic protein (BMP) signaling pathways. As another candidate gene, the protein encoded by BMP8B activates the nuclear translocation of SMAD4, which regulates the expression of genes associated with the differentiation of primordial germ cells or testicular development. Additionally, BMP4, a member of the BMP family, regulates the interaction between metanephric mesenchyme and ureteric buds by suppressing GDNF. Conclusions: Taken together, our findings suggested that the development of the kidney and urinary tract is intimately linked with that of male reproductive organs via BMP/SMAD signaling pathways.

In this study, we investigated the feasibility and the benefit of using the extraperitoneal (EP) approach with six trocars over a 4-cm distance in a flat or 5 degrees Trendelenburg position. We also introduced four new steps to the surgical procedure and compared the surgical results and complications between the EP and TP approach using propensity score matching. Methods: Between August 2012 and August 2016, 200 consecutive patients without any physical restrictions underwent RARP with the EP approach in a less than 5 degrees Trendelenburg position, and 428 consecutive patients underwent RARP with the TP approach in a steep Trendelenburg position. Four new steps to RARP using the EP approach were developed: 1) arranging six trocars; 2) creating the EP space using laparoscopic forceps; 3) holding the separated prostate in the EP space outside the robotic view; and 4) preventing a postoperative inguinal hernia. Clinicopathological results and complications were compared between the EP and TP approaches using propensity score matching. Propensity scores were calculated for each patient using multivariate logistic regression based on the preoperative covariates. Results: All 200 patients safely underwent RARP using the EP approach. The mean volume of estimated blood loss and duration of indwelling urethral catheter use were significantly lower with the EP approach than the TP approach (139.9 vs 184.9 mL, p = 0.03 and 5.6 vs 7.7 days, p < 0.01, respectively). No significant differences in the positive surgical margin were observed. None of the patients developed an inguinal hernia postoperatively after we introduced this technique. Conclusions: The EP approach to RARP was safely performed regardless of patient physique or contraindications to a steep Trendelenburg position. Our method, which involved using the EP approach to perform RARP, can decrease the amount of perioperative blood loss, the duration of indwelling urethral catheter use, and the incidence of postoperative inguinal hernia development.

We previously reported the potential therapeutic role of osteopontin (OPN) and macrophages in CaOx kidney stone formation, discovered using genome-recombined mice and genome-wide analyses. Here, to characterize the genetic pathogenesis of RPs, we used microarrays and immunohistology to compare gene expression among renal papillary RP and non-RP tissues of 23 CaOx stone formers (SFs) (age- and sex-matched) and normal papillary tissue of seven controls. Transmission electron microscopy showed OPN and collagen expression inside and around RPs, respectively. Cluster analysis revealed that the papillary gene expression of CaOx SFs differed significantly from that of controls. Disease and function analysis of gene expression revealed activation of cellular hyperpolarization, reproductive development, and molecular transport in papillary tissue from RPs and non-RP regions of CaOx SFs. Compared with non-RP tissue, RP tissue showed upregulation (>2-fold) of LCN2, IL11, PTGS1, GPX3, and MMD and downregulation (0.5-fold) of SLC12A1 and NALCN (P<0.01). In network and toxicity analyses, these genes associated with activated mitogenactivated protein kinase, the Akt/phosphatidylinositol 3-kinase pathway, and proinflammatory cytokines that cause renal injury and oxidative stress. Additionally, expression of proinflammatory cytokines, numbers of immune cells, and cellular apoptosis increased in RP tissue. This study establishes an association between genes related to renal dysfunction, proinflammation, oxidative stress, and ion transport and RP development in CaOx SFs.

Here, we investigated the role of M1/M2s in crystal development by using in vitro and in vivo approaches. The crystal phagocytic rate of bone marrow-derived M2M phi s was higher than that of bone marrow-derived M phi s and M1M phi s and increased on co-culture with renal tubular cells (RTCs). However, the amount of crystal attachment on RTCs reduced on co-culture with M2M phi s In six hyperoxaluric C57BL/6J mice, M1M phi transfusion and induction by LPS and IFN-gamma facilitated renal crystal formation, whereas M2M phi transfusion and induction by IL-4 and IL-13 suppressed renal crystal formation compared with the control. These M2M phi treatments reduced the expression of crystal-related genes, such as osteopontin and CD44, whereas M1M phi treatment increased the expression of pro-inflammatory and adhesion-related genes such as IL-6, inducible NOS, TNF-alpha, C3, and VCAM-1. The expression of M2M phi-related genes was lower whereas that of M1M phi-related genes was higher in papillary tissue of CaOx stone formers. Overall, our results suggest that renal crystal development is facilitated by M1M phi s, but suppressed by M2M phi s.

A majority of pediatric RMS cases involve the embryonal type and occur in the soft tissues of the head and neck or the urogenital organs, which contain paratesticular tissues. We report herein two cases of pediatric paratesticular RMS. One case was embryonal, whereas the other case was alveolar; the latter exhibited PAX7-FOXO1 gene chimerism and rapid progression. Notably, this is the first report of pediatric paratesticular pure-type alveolar RMS in Japan. (C) 2016 Elsevier Inc.

As it has only been reported in three cases, the clinical manifestations remain unclear. We report a case of hemiscrotal agenesis with ipsilateral cryptorchidism. Based on the thermal assessment of the scrotum, concurrent scrotoplasty and orchiopexy were carried out, and the scrotoplasty improved the thermal environment of the fixed left testis. Furthermore, the low expression of androgen receptor and steroid-5-alpha-reductase, alpha polypeptide 2 in the affected side of the scrotum likely resulted in the characteristics of absent scrotal rugae, and pigmentation on histological and biological analyses. For future fertility, we believe that scrotoplasty should be considered as a management option for hemiscrotal agenesis.

MethodsA total of 15 ureters from nine patients (age range 2-25 years) underwent robot-assisted laparoscopic extravesical ureteral reimplantation for the management of vesicoureteral reflux. The reflux was classified as grade I in one ureter, grade II in two ureters, grade III in seven ureters, grade IV in three ureters and grade V in two ureters. One of the five female patients had a bilateral duplex system, and reflux was observed in all four ureters. The da Vinci surgical system was utilized. Ureteral advancement was carried out in all cases. We also compared the operative outcomes between conventional laparoscopic procedure and robotic surgery. ResultsThe console time was 211.587.4min (median +/- standard deviation) in the bilateral cases and 144.0 +/- 40.8min in the unilateral cases. Urethral catheters were removed at one or two postoperative days. None of the patients suffered postoperative complications, such as urine leakage or urinary retention. Postoperative voiding cystourethrography showed that the reflux had been resolved in 14 of the 15 ureters (success rate 93.3%). In the remaining case, the reflux grade decreased from III to I. The operative outcomes of robotic surgery were favorable and safe compared with conventional laparoscopic procedure. ConclusionsOur preliminary results showed that robot-assisted laparoscopic surgery is a feasible and useful approach to extravesical ureteral reimplantation, even for patients with bilateral reflux.

MethodsThe study cohort comprised 708 consecutive male patients with clinically localized prostate cancer who underwent laparoscopic radical prostatectomy (n=551) or robot-assisted radical prostatectomy (n=157) between January 1999 and September 2012. Operative time, estimated blood loss, complications, and positive surgical margins frequency were compared between laparoscopic radical prostatectomy and robot-assisted radical prostatectomy. ResultsThere were no significant differences in age or body mass index between the laparoscopic radical prostatectomy and robot-assisted radical prostatectomy patients. Prostate-specific antigen levels, Gleason sum and clinical stage of the robot-assisted radical prostatectomy patients were significantly higher than those of the laparoscopic radical prostatectomy patients. Robot-assisted radical prostatectomy patients suffered significantly less bleeding (P<0.05). The overall frequency of positive surgical margins was 30.6% (n=167; 225 sites) in the laparoscopic radical prostatectomy group and 27.5% (n=42; 58 sites) in the robot-assisted radical prostatectomy group. In the laparoscopic radical prostatectomy group, positive surgical margins were detected in the apex (52.0%), anterior (5.3%), posterior (5.3%) and lateral regions (22.7%) of the prostate, as well as in the bladder neck (14.7%). In the robot-assisted radical prostatectomy patients, they were observed in the apex, anterior, posterior, and lateral regions of the prostate in 43.0%, 6.9%, 25.9% and 15.5% of patients, respectively, as well as in the bladder neck in 8.6% of patients. ConclusionsPositive surgical margin distributions after robot-assisted radical prostatectomy and laparoscopic radical prostatectomy are significantly different. The only disadvantage of robot-assisted radical prostatectomy is the lack of tactile feedback. Thus, the robotic surgeon needs to take this into account to minimize the risk of positive surgical margins.

We performed genome wide analysis in patients with 46, XX testicular disorders of sex development to comprehensively elucidate the mechanisms of testicular differentiation. Materials and Methods: Whole genomic DNA was extracted from the peripheral blood of 4 patients with 46, XX testicular disorders of sex development who were SRY negative. Genomic DNA was hybridized to a GeneChip (R) human mapping 250K array set. Compared to normal female data, we detected common loss of heterozygosity and copy number variation regions in 4 patients using Genotyping Console (TM) software. Results: Loss of heterozygosity was detected in 19 regions of 11 chromosomes. A total of 27 genes or nearby genomic areas were included in the applicable regions. Copy number loss was recognized in 13 regions of 10 chromosomes, and these regions included 55 genes. Copy number gain was detected in 6 regions of 4 chromosomes, which included the upstream region of the SOX3 gene. Conclusions: The regions with loss of heterozygosity did not contain genes associated with testicular differentiation. However, the upstream area of the SOX3 gene, which is located in Xq27.1, was included in the region of copy number gain. These results suggest that high expression of the SOX3 gene led to testicular differentiation despite SRY gene loss. As this applicable area is not within a coding region, genome wide analyses were valuable for detecting the novel regions associated with testicular differentiation.

M phi s can be inflammatory (M1) or anti-inflammatory (M2), and colony-stimulating factor-1 (CSF-1) mediates polarization to the M2M phi phenotype. M2M phi s promote renal tissue repair and regeneration, but it is not clear whether these cells are involved in suppressing renal crystal formation. We investigated the role of M2M phi s in renal crystal formation during hyperoxaluria using CSF-1-deficient mice, which lack M2M phi s. Compared with wild-type mice, CSF-1-deficient mice had significantly higher amounts of renal calcium oxalate crystal deposition. Treatment with recombinant human CSF-1 increased the expression of M2-related genes and markedly decreased the number of renal crystals in both CSF-1-deficient and wild-type mice. Flow cytometry of sorted renal M phi s showed that CSF-1 deficiency resulted in a smaller population of CD11b(+)F4/80(+)CD163(+)CD206(hi) cells, which represent M2-like M phi s. Additionally, transfusion of M2M phi s into CSF-1 deficient mice suppressed renal crystal deposition. In vitro phagocytosis assays with calcium oxalate monohydrate crystals showed a higher rate of crystal phagocytosis by M2-polarized M phi s than M1-polarized M phi s or renal tubular cells. Gene array profiling showed that CSF-1 deficiency resulted in disordered M2- and stone-related gene expressions. Collectively, our results provide compelling evidence for a suppressive role of CSF-1 signaling in renal crystal formation.

We previously reported attenuated spermatogonial stem cell activity in cryptorchid testes, which resulted in altered spermatogenesis and affected fertility. However, few groups have examined the differentiation process from gonocytes to spermatogonial stem cells. To clarify the underlying mechanisms comprehensively we performed microarray analysis to assess differential expression of transcripts between normal and undescended testes in juvenile rats. Materials and Methods: Using microarray analysis we compared whole mRNA expression of normal and cryptorchid testes in a rat model. We subsequently validated differential expression of candidate genes by real-time reverse transcriptase-polymerase chain reaction and performed immunohistochemistry. We also investigated the methylation status of histone H3K4 in cryptorchid testes and the GC-1 spermatogonial cell line. Results: We detected 24 up-regulated and 39 down-regulated genes. Of these genes Kdm5a expression was significantly higher in undescended testes. Immunohistochemistry showed that Kdm5a was localized in the nuclei of gonocytes, spermatogonia and spermatocytes. H3K4me2/me3 expression levels were decreased in undescended testes at 9 days postpartum. Furthermore, Kdm5a over expression in GC-1 cells led to increased expression of Esr2, Neurog3, Pou5f1, Ret and Thy1. Conclusions: Recent investigations revealed that not only genetic but also epigenetic regulation has a role in spermatogenesis. Kdm5a is likely involved in the transformation of gonocytes into spermatogonial stem cells by transcriptional regulation of specific genes via H3K4 histone modification. To our knowledge this is the first report of epigenetic analysis of germ cell differentiation during early spermatogenesis.

Materials and Methods: Using microarray analysis we compared total miRNA expression in unilateral undescended testes with that in contralateral descended and normal testes in a rat model of cryptorchidism. The model was derived by administering flutamide to pregnant Sprague Dawley (R) rats. We identified mRNA targets of miRNAs by bioinformatic analysis, followed by in situ hybridization and immunohistochemistry to localize candidate miRNAs and mRNAs, respectively. We also investigated whether miRNAs could inhibit target protein expression in vitro. Results: Microarray analysis and subsequent quantitative reverse transcriptase-polymerase chain reaction showed that only miR-135a was expressed at a lower level in undescended testes. We identified its target as FoxO1, which is essential for stem cell maintenance. miR-135a and FoxO1 localized to spermatogonial stem cells. FoxO1 localized to the spermatogonial stem cell nucleus less frequently in undescended testes, indicating that the activity of FoxO1, which acts as a transcription factor, is altered in undescended testes. Finally, miR-135a transfection into spermatogonia in vitro resulted in down-regulation of FoxO1 expression. Conclusions: In cryptorchid testes there is a decreased number of spermatogonial stem cells in which FoxO1 is activated, indicating that failure of spermatogonial stem cell maintenance results in spermatogenesis alteration. We also noted interaction between miR-135a and FoxO1, and propose that miR-135a contributes to spermatogonial stem cell maintenance through modulation of FoxO1 activity.

Patients and Methods: From December 2010 to May 2012, all patients with an exophytic, solitary, enhancing small (4.0cm) renal mass were chosen to receive the study treatment. A multichannel port provided both a retroperitoneal and transperitoneal approach. A rigid, articulating apparatus was used to perform dissection, exposure of tumor, tissue coagulation, and resection without hilar clamping. Pathologic and hematologic data, subjective evaluation of pain, and scar appearance were analyzed. Results: Nonhilar clamp LESS partial nephrectomy by using a microwave tissue coagulator was performed in seven patients (mean operative time, 208min; mean blood loss, 39mL; mean renal mass size, 1.7cm); one procedure with uncontrolled bleeding needed to be converted to conventional laparoscopic partial nephrectomy. No transfusion was necessary. Pathologic investigation demonstrated six renal-cell carcinomas and one oncocytoma. The hemoglobin level decreased by a mean of 1.1g/dL. Patients did not complain about pain, and they had great satisfaction with the results. The mean duration of hospital stay was 13.8 days. Conclusions: LESS partial nephrectomy without hilar clamping by using a microwave tissue coagulator is possible for renal masses, confers postoperative outcomes comparable to the standard counterpart, and assures patient satisfaction. By use of a microwave tissue coagulator, additional trocars were not necessary, and LESS partial nephrectomy could be accomplished through a single port, which reduced invasion and increased the cosmetic satisfaction of the patients.

The development of the minimally invasive PCNL (mini-PCNL) has reduced the complications of the surgery; it also appears to be associated with less morbidity than the conventional PCNL (con-PCNL). This study aimed at evaluating the efficacy of endoscopic intrarenal surgery, using the prone-split leg position, using flexible ureteroscopy and mini-PCNL (mini- endoscopic combined intrarenal surgery [ECIRS]) by retrospectively comparing this technique with mini-PCNL and con-PCNL. Patients and Methods: In total, 161 consecutive patients who were observed for the follow-up of large renal calculi between February 2004 and January 2013 were selected for mini-ECIRS (60), mini-PCNL (19), or con-PCNL (82). Mini-ECIRS was performed with patients in the prone split-leg position via 18F minipercutaneous tract and 14F ureteral access sheath. The mini-PCNL was performed via 18F percutaneous tract and con-PCNL performed via the 30F tract. Mean size of the renal calculi removed via mini-ECIRS, mini-PCNL, and con-PCNL were 39.2, 38.4, and 34.6mm, respectively. Results: Average surgical time for mini-ECIRS was shorter than that for mini-PCNL and con-PCNL (120.5 vs 181.9 vs 134.1min, respectively; P<0.001). The stone-free rate for mini-ECIRS was significantly higher than that of the other procedures (initial rates 81.7% vs 38.9% vs 45.1%, respectively; P<0.001; rates after further treatment 86.7% vs 61.1% vs 61.0%, respectively; P=0.002). Only one patient in the mini-ECIRS group needed blood transfusions. The decrease in hemoglobin during mini-ECIRS and mini-PCNL was significantly lower than that during con-PCNL (P=0.011). Conclusion: Mini-ECIRS is better than monotherapy with mini-PCNL or con-PCNL. The study results show that mini-ECIRS is a safe, efficient, and versatile procedure that can be effective for the management of renal calculi.

Methods: This retrospective study examined prospectively collected data on 219 robotic-assisted laparoscopic radical prostatectomies performed from May 2011 to February 2013. Patients were divided into four groups based on pathologic prostate weight: < 30 g, 30-49 g, 50-79 g, and >= 80 g. Continence and sexual function were assessed using validated questionnaires. Results: Of the 219 patients, 19, 143, 51, and 6 had prostates weighing < 30 g, 30-49 g, 50-79 g, and >= 80 g, respectively. Significant differences were found between the preoperative Gleason scores, total operative times, and robotic times of the groups. Both estimated blood loss and anastomosis time tended to be greater in the higher prostate weight groups, but the differences were not significant. No significant differences were observed in transfusion rate, length of catheterization, complication incidence, or positive surgical margins. The return of urinary function, as determined by questionnaire scores, was not affected by prostate weight. Conclusions: Robotic-assisted laparoscopic radical prostatectomy can be performed safely and with similar perioperative outcomes, regardless of prostate weight. Indeed, oncological outcome, urinary continence, and complications were similar across the prostate weight groups, suggesting that robotic-assisted laparoscopic radical prostatectomy with a posterior approach to the seminal vesicle may be performed effectively on men with large prostates, despite greater surgical times.

Here, we investigated the association of these loci with nephrolithiasis by using an independent Japanese sample set. We performed case-control association analysis using 601 patients with nephrolithiasis and 201 control subjects. We selected seven single-nucleotide polymorphisms (SNPs): rs12654812 and rs11746443 from 5q35.3 (RGS14-SLC34A1-PFN3-F12); rs12669187 and rs1000597 from 7p14.3 (INMT-FAM188B-AQP1); and rs7981733, rs1170155, and rs4142110 from 13q14.1 (DGKH (diacylglycerol kinase)), which were previously reported to be significantly associated with nephrolithiasis. rs12654812, rs12669187 and rs7981733 were significantly associated with nephrolithiasis after Bonferroni's correction (P=3.12 x 10(-3), odds ratio (OR)=1.43; P=6.40 x 10(-3), OR=1.57; and P=5.00 x 10(-3), OR=1.41, respectively). Meta-analysis of current and previous GWAS results indicated a significant association with nephrolithiasis (P=7.65 x 10(-15), 7.86 x 10(-14) and 1.06 x 10(-9), respectively). We observed a cumulative effect with these three SNPs; individuals with three or more risk alleles had a 5.9-fold higher risk for nephrolithiasis development than those with only one risk allele. Our findings elucidated the significance of genetic variation at these three loci in nephrolithiasis in the Japanese population.

Recently, a few submicroscopic genomic rearrangements have been reported as novel genetic causes of 46, XY DSD. Methodology/Principal Findings: To clarify the role of cryptic rearrangements in the development of 46, XY DSD, we performed array-based comparative genomic hybridization analysis for 24 genetic males with genital abnormalities. Heterozygous submicroscopic deletions were identified in three cases (cases 1-3). A similar to 8.5 Mb terminal deletion at 9p24.1-24.3 was detected in case 1 that presented with complete female-type external genitalia and mental retardation; a similar to 2.0 Mb interstitial deletion at 20p13 was identified in case 2 with ambiguous external genitalia and short stature; and a similar to 18.0 Mb interstitial deletion at 2q31.1-32 was found in case 3 with ambiguous external genitalia, mental retardation and multiple anomalies. The genital abnormalities of case 1 could be ascribed to gonadal dysgenesis caused by haploinsufficiency of DMRT1, while those of case 3 were possibly associated with perturbed organogenesis due to a deletion of the HOXD cluster. The deletion in case 2 affected 36 genes, none of which have been previously implicated in sex development. Conclusions/Significance: The results indicate that cryptic genomic rearrangements constitute an important part of the molecular bases of 46, XY DSD and that submicroscopic deletions can lead to various types of 46, XY DSD that occur as components of contiguous gene deletion syndromes. Most importantly, our data provide a novel candidate locus for 46, XY DSD at 20p13.

Methods: The data of 12 patients (7 men and 5 women; mean age, 68.5 +/- 8.9 years) with large pyeloureteral junction (2 cases) and upper ureteral (10 cases) stones (25.3 +/- 7.4 mm) that had undergone retroperitoneal laparoscopic ureterolithotomy were reviewed. Renal function was analyzed by the estimated glomerular filtration rate (eGFR) and renal scintigraphy using 99mTc-mercaptoacetyltriglycine (99mTc-MAG3) before and 3 months after surgery. Results: The mean operative time was 129.5 +/- 21.4 minutes, with a mean blood loss of 64.4 +/- 78.2 mL. The mean duration of hospital stay after surgery was 6.4 +/- 2.7 days, and the mean duration of stenting was 7.2 +/- 1.7 weeks. A stone clearance rate of 100% was achieved, and no patient developed ureteric stricture. 99mTc-MAG3 scintigraphy showed that laparoscopic removal of calculi did not affect renal function, but did improve ureteral occlusion. Conclusions: Retroperitoneal laparoscopic ureterolithotomy is a safe and effective treatment option for reducing ureteral obstruction in select patients with large proximal ureteric stones.

In this study, individual variation in the genetic response to low-dose bisphenol A (BPA) was investigated in human foreskin fibroblast cells (hFFCs) derived from child cryptorchidism (CO) and hypospadias (HS) patients. Methodology/Principal Findings: hFFCs were collected from control children without MGMs (n = 5) and child CO and HS patients (n = 8 and 21, respectively). BPA exposure (10 nM) was found to inhibit matrix metalloproteinase-11 (MMP11) expression in the HS group (0.74-fold, P = 0.0034) but not in the control group (0.93- fold, P = 0.84) and CO group (0.94-fold, P = 0.70). Significantly lower levels of MMP11 expression were observed in the HS group compared with the control group (0.80-fold, P = 0.0088) and CO group (0.79-fold, P = 0.039) in response to 10 nM BPA. The effect of single-nucleotide polymorphism rs5000770 (G. A), located within the aryl hydrocarbon receptor nuclear translocator 2 (ARNT2) locus, on individual sensitivity to low-dose BPA was investigated in the HS group. A significant difference in neurotensin receptor 1 (NTSR1) expression in response to 10 nM BPA was observed between AA and AG/GG groups (n = 6 and 15, respectively. P = 0.031). However, no significant difference in ARNT2 expression was observed (P = 0.18). Conclusions/Significance: This study advances our understanding of the specificity of low-dose BPA effects on human reproductive health. Our results suggest that genetic variability among individuals affects susceptibility to the effects of EEDs exposure as a potential cause of HS.

However, ureteral advancement with this technique has not been attempted. We examined the usefulness of ureteral advancement via laparoscopy. Materials and Methods: A total of 30 patients with 51 refluxing ureters underwent laparoscopic extravesical ureteral reimplantation for treatment of vesicoureteral reflux between August 2009 and September 2011. Mean +/- SD patient age was 60.8 +/- 48.6 months. During the procedure 15 patients underwent ureteral advancement (advancement group), while 15 did not (nonadvancement group). We compared operative times and postoperative rates of urinary tract infections and persistent reflux between the groups. Results: There was no significant difference in operative times in unilateral (mean +/- SD 110 +/- 25 vs 125 +/- 42 minutes) and bilateral cases (mean +/- SD 214 +/- 52 vs 203 +/- 40 minutes) between the nonadvancement vs advancement groups. All patients underwent voiding cystourethrography 3 to 4 months postoperatively. Reflux resolution rate for ureters was significantly higher in the advancement group (100%) than in the nonadvancement group (85%, p +/- 0.05). No patient in the advancement group had postoperative urinary tract infection. Fixation of the ureter with the bladder muscularis at the proximal limit of the detrusor defect and/or a percutaneous hitch stitch placed in the ventral side of the proximal limit of the detrusor defect facilitated ureteral advancement. Conclusions: Ureteral advancement is a simple and feasible procedure in laparoscopic ureteral extravesical reimplantation and may improve the resolution rate of vesicoureteral reflux.

We report the case of a 7-year-old girl with Turner's syndrome with 45,XO/46,XY mosaicism, for whom bilateral prophylactic gonadectomy using LESS was performed. Histopathological findings revealed bilateral streak gonads. The surgical and cosmetic outcome was excellent. Diagnostic and therapeutic laparoscopy is essential for accurate clinical management of patients with disorders of sex development. Although this is only a single case report, it supports the theory that LESS is an exceedingly practical and superior technique for such children, since it provides excellent magnification, as well as allowing normal psychological development owing to the concealed scar. In this study, we explored for association between 384 SNPs in 15 genes (AHR, AHRR, ARNT, ARNT2, NR1I2, RXRA, RXRB, RXRG, CYP1A1, CYP1A2, CYP1B1, CYP2B6, CYP3A4, CYP17A1 and CYP19A1) and risk of cryptorchidism (CO) and hypospadias (HS) in 334 Japanese (JPN) males (141 controls, 95 CO and 98 HS) and 187 Italian (ITA) males (129 controls and 58 CO). In the JPN study group, five SNPs from ARNT2 (rs2278705 and rs5000770), CYP1A2 (rs2069521), CYP17A1 (rs4919686) and NR1I2 (rs2472680) were significantly associated at both allelic and genotypic levels with risk of at least one genital malformation phenotype. In the ITA study group, two SNPs in AHR (rs3757824) and ARNT2 (rs1020397) were significantly associated with risk of CO. Interaction analysis of the positive SNPs using multifactor dimensionality reduction demonstrated that synergistic interaction between rs2472680, rs4919686 and rs5000770 had 62.81% prediction accuracy for CO (P=0.011) and that between rs2069521 and rs2278705 had 69.98% prediction accuracy for HS (P=0.001) in JPN population. In a combined analysis of JPN and ITA population, the most significant multi-locus association was observed between rs5000770 and rs3757824, which had 65.70% prediction accuracy for CO (P=0.055). Our findings indicate that genetic polymorphisms in genes involved in EED metabolism are associated with risk of CO and HS. On the 16th day after birth, complete primary exstrophy repair was performed. The bladder tissue obtained during surgery contained collagen tissue and a little smooth muscle, as well as a few neuron cells and kit-positive interstitial cells in the submucosal layer. This is the first report of a histopathologic analysis of the bladder tissue in cloacal exstrophy. We also discuss the influence of the pathologic changes on bladder dysfunction. Differentially expressed genes were identified using an unpaired Student's t test with P value cut off at 0.05 and fold change of more than 1.2. These genes were selected for network generation and pathway analysis using Ingenuity Pathways Analysis, Pathway Express and KegArray. Seventy-one genes (42 downregulated and 29 upregulated) were identified as significantly differentially expressed in response to BPA, among which 43 genes were found to be affected exclusively by BPA compared with E2 and TCDD. Of particular interest, real-time PCR analysis revealed that the expression of matrix metallopeptidase 11 (MMP11), a well-known effector of development and normal physiology, was found to be inhibited by BPA (0.47-fold and 0.37-fold at 10 nM and 100 nM, respectively). Furthermore, study of hFFCs derived from HS and cryptorchidism (CO) patients (n = 23 and 11, respectively) indicated that MMP11 expression was significantly lower in the HS group than in the CO group (0.25-fold, P = 0.0027). Conclusions/Significance: This present study suggests that an involvement of BPA in the etiology of HS might be associated with the downregulation of MMP11. Further study to elucidate the function of the novel target genes identified in this study during genital tubercle development might increase our knowledge of the effects of low-dose BPA exposure on human reproductive health.

Methods: A total of 102 boys with vanishing testis were treated in our hospital from 1984 to 2011. Of these cases, 91 testicular nubbins were excised. Immunohistochemical analysis of testicular nubbins was carried out using anti-Wilms tumor 1 antibody, which is a stable marker of Sertoli cells. Reverse transcription polymerase chain reaction was also carried out using Wilms tumor 1-specific primers. Results: Most testicular nubbins were associated with inguinal lesion (51 patients, 56.0%). Light microscopy showed that 11 patients (12.5%) had seminiferous tubules (with germ cells in three patients [3.4%]), and 77 patients lacked seminiferous tubules, some of which had calcification (26 patients, 29.5%), and/or deposition of hemosiderin (21 patients, 23.9%). Immunohistochemical analysis showed Wilms tumor 1 expression not only in the Sertoli cells of the seminiferous tubules in the seminiferous tubule-positive patients, but also in the interstitium of testicular nubbins in the seminiferous tubule-negative patients. Reverse transcription polymerase chain reaction showed Wilms tumor 1 messenger ribonucleic acid expression in the testicular nubbins of both seminiferous tubule-positive and tubule-negative patients. Conclusions: The presence of Wilms tumor 1-positive cells in the interstitium of vanishing testis lacking seminiferous tubules suggests that the disturbance of testicular development after Sertoli cell differentiation and during testicular tubule formation might be involved in the etiology of vanishing testis.

MATERIALS AND METHODS Pregnant Sprague - Dawley rats were administered fl utamide (7.5 mg) on gestational days 15 - 17 to produce hypospadiac pups. Dams were killed on gestational day 17, and the genital tubercles (GTs) of male pups were harvested. Gene expression of RNA isolated from the GTs was analysed using an oligonucleotide microarray containing 20 500 genes. The results of microarray analysis were confi rmed using quantitative real- time PCR (qPCR). Protein expression levels were studied using Western blot analysis. The distribution of genes associated with GT development in rats was histologically examined. Prepuces harvested from patients with hypospadias and phimosis were immunohistochemically examined for gene distribution. RESULTS Of the 20 500 genes, 23 annotated genes, including prolactin-induced protein (Pip), in the GTs of the hypospadiac rats were expressed at levels less than half of that of similar genes in the GTs of the control rats. Findings from qPCR and Western blot analysis revealed signifi cantly lower Pip / PIP expression in the GTs of the hypospadiac rats than in those of the control rats. Immunohistochemical analysis revealed PIP expression in the prepuces of the GTs of the control and hypospadiac rats. PIP was expressed in the human prepuces of the patients with hypospadias and phimosis. CONCLUSIONS Pip / PIP, expressed at low levels in the GTs of hypospadiac rats, may be associated with preputial development. This model can be useful to elucidate the molecular mechanisms underlying penile and urethral development as well as preputial development. Further studies should provide detailed information regarding the molecular aetiology of hypospadias.

Here, we present 3 cases of KS with detailed description. In Case 1, testicular morphology was examined by testicular biopsy, and Leydig cells were examined by immunohistochemistry using antibodies against Ad4BP/SF1. Contrary to our predictions, the present study revealed the presence of Leydig cells in the testis. Testicular morphology in the patients with KS is more varied than expected, and further investigation is required to elucidate hormonal effects on normal testicular development. Materials and Methods: Histological findings and expression patterns of the stem cell marker undifferentiated embryonic cell transcription factor 1 were examined in a unilateral cryptorchid rat model. We removed unilateral descended testis and contralateral descended testis from cryptorchid and normal rats (control), respectively, 18 days postcoitum to 144 days postpartum. Results: In descended testes gonocyte differentiation into early A spermatogonia occurred at 9 days postpartum. However, this transformation was altered in undescended testes. Furthermore, the undifferentiated embryonic cell transcription factor 1 negative early A spermatogonia-to-positive early A spermatogonia ratio was significantly higher in the undescended testis group (mean +/- SD 0.69 +/- 0.04) than in the control (0.46 +/- 0.10, p = 0.037) and descended testis (0.44 +/- 0.05, p = 0.022) groups, indicating decreased early A spermatogonia with spermatogonial stem cell activity in cryptorchid testes. Conclusions: In cryptorchid testes the differentiation from gonocytes into early A spermatogonia and the stem cell activity of early A spermatogonia were altered during the early stage of spermatogenesis, suggesting that the loss of spermatogonial stem cell activity in cryptorchid rats resulted in altered spermatogenesis, thus interfering with fertility.

MATERIALS AND METHODS Fourteen rats were divided into a torsion group (n = 7) and a control group with sham operation (n = 7). Torsion was created by 720 rotation of the left testis in a counter-clockwise direction. Using a handheld compartment monitor, the ITP of the torsed testes was measured three times: before torsion (pre-torsion), just before torsion repair (pre-detorsion) and 5 min after torsion repair (post-detorsion). We evaluated the correlation between ITP and testicular weight, epididymal sperm count or pathological findings, such as the seminiferous tubule diameter (STD) and the modified Johnsen's score, 4 weeks after surgery. RESULTS Mean (se) pre-torsion, pre-detorsion and post-detorsion ITP values in the torsion group were 5.9 (2.5), 19.7 (10.7) and 8.2 (4.8) cm H(2)O, respectively. The ITP in torsed testes significantly increased after torsion (P < 0.01) and decreased after detorsion (P < 0.01). Strong correlations were observed between the reduction of ITP after detorsion and testicular weight (r = 0.87, P < 0.05), epididymal sperm count (r = 0.94, P < 0.05), STD (r = 0.87, P < 0.05) or the Johnsen's score (r = 0.99, P < 0.001). CONCLUSION A smaller reduction in ITP after detorsion can be a risk factor for subsequent disturbance of spermatogenesis, suggesting that ITP can be an index for determining whether the affected testis should be removed after testicular torsion.

Examination disclosed a large tumor in the right kidney and transabdominal radical nephrectomy was performed. Histopathological study demonstrated clear cell sarcoma of the kidney (CCSK). Cytogenetic analyses performed with blood lymphocytes showed translocation (1;6) (p32.3;q21). This is the first case of CCSK showing chromosomal translocation in blood lymphocytes. In addition, molecular analyses demonstrated high expression levels of EGFR and KIT proteins in the tumor tissue of CCSK. There has been a shift from routine surgical removal to nonoperative management. However, recently, an argument has emerged with regard to the management of MCDK, with several studies recommending surgical removal. We report a case of pelvic MCDK that was diagnosed and subsequently removed by a laparoscopic procedure. A laparoscopic approach to the renal and pelvic areas is a minimally invasive, safe, and effective procedure to diagnose and treat MCDK. To discuss the potential role of KIT-positive interstitial cells of Cajal (ICC)-like cells in the urinary bladder. MATERIALS AND METHODS Reverse transcriptase-polymerase chain reaction and western blotting was used to confirm the absence of c-kit mRNA and protein in the bladders of 12-week-old WsRCWs/Ws rats. Light and transmission electron microscopy was used to identify the differences in morphological and ultrastructural characteristics of the bladder between WsRCWs/Ws and wild-type (WsRC+/+) rats. The voiding pattern of WsRCWs/Ws rats and the effects of cyclophosphamide (CYP) and protamine sulphate on bladder function were examined using cystometry. RESULTS In WsRC+/+ rats, c-kit mRNA and KIT protein expression were observed in the urinary bladder, while they were not detectable in WsRCWs/Ws rats. Deformation of ICC-like cells with the collapse of the organelle was not observed in the bladders of WsRCWs/Ws rats. Each cystometry variable in WsRCWs/Ws rats was similar to that in WsRC+/+ rats. The reduction in the intercontraction intervals in WsRCWs/Ws rats with chemically (CYP and protamine sulphate) induced cystitis was significantly lower than in WsRC+/+ rats (P < 0.05). CONCLUSION Certain voiding disturbances might be associated with impaired KIT signalling in ICC-like cells, therefore, KIT could be a candidate target for medical therapy in the future.

We predicted testicular function in prepubertal boys with a history of cryptorchidism and/or hypospadias by measuring serum hormone levels. Materials and Methods: A total of 82 prepubertal boys who underwent orchiopexy and/or hypospadias repair in childhood were enrolled in the study. Patients were surgically treated for cryptorchidism (23 in group 1), hypospadias (49 in group 2), cryptorchidism and hypospadias (10 in group 3), and hydrocele testis (7 in control group 4). Serum hormones, including luteinizing hormone, follicle-stimulating hormone and total testosterone, were measured separately by age less than 12.5, 12.5 to 13.5 and greater than 13.5 years, and by Tanner pubertal stage. Results: Follicle-stimulating hormone in group 3 was significantly higher than in groups 1, 2 and 4 at ages 12.5 to 13.5 and greater than 13.5 years, and for Tanner stages 2 and 3 (p < 0.05). However, luteinizing hormone and testosterone did not differ among the groups regardless of age or Tanner stage. Group 3 patients had significantly higher follicle-stimulating hormone regardless of the severity of cryptorchidism or hypospadias. Conclusions: Data suggest that testicular function in patients with cryptorchidism plus hypospadias is more severely impaired than that in patients with cryptorchidism or hypospadias, lending clinical support to the testicular dysgenesis syndrome hypothesis of a common origin.

The aim of the present study was to elucidate the current status of perioperative care for these treatments by carrying out a nationwide survey. Methods: Assisted by the Japanese Endourology and ESWL Association, perioperative data from 157 institutions participating in this survey were collected and analyzed. Results: This survey included 3918 patients undergoing TURP, 242 TUR in saline (TURis), 638 holmium laser enucleation of the prostate (HoLEP), 90 holmium laser ablation (HoLAP) and 241 photoselective vaporization (PVP). Mean operative time was shorter in TURP (71 min) and longer in HoLEP (127). Although no transfusions were required in cases undergoing HoLAP or PVP, blood was frequently transfused in those undergoing TURis (25.6%), TURP (10.2%) and HoLEP (7.8%), and the difference was significant. During the hospital stay, the incidence of TUR-syndrome, postoperative bleeding requiring bladder irrigation, acute urinary retention/difficulty on micturition and pad use at discharge was highest in TURP (2.3%), TURis (7.9%), HoLAP (16.7%) and HoLEP (15.1%), respectively. Two patients undergoing TURP died (0.05%). The shortest mean postoperative hospital stay was for PVP (1.6 days, even if the readmission rate within 90 days was the highest in this same group; 6.2%). Perioperative care during hospital stay varied among the five types of procedures. Conclusions: This survey provides useful documentation on the current status of minimally invasive treatments for BPH in Japan. Complication rates for TURP are not significantly higher as compared with other procedures. Thus, TURP can still be considered as the gold standard for BPH treatment.

Patients and Methods: Of the 53 boys (mean age, 24.5 months), nonpalpable testis was on the right side in 13, on the left in 34, and bilateral in 6. Three boys with nonpalpable testis involved contralateral palpable undescended testis. We applied only laparoscopic treatment without open inguinal incision for all cases. After laparoscopic orchiopexy and orchiectomy, we performed subsequent laparoscopic inguinal ring and/or peritoneal defect closure to prevent postoperative inguinal hernia formation, and concurrent laparoscopic repair for cases of contralateral undescended testes and open processus vaginalis under the same anesthesia. Results: All boys underwent laparoscopic treatment immediately after laparoscopic evaluation. We performed laparoscopic orchiopexy for 22 cases with intra-abdominal testes. Twenty-seven boys underwent laparoscopic groin exploration, and 24 underwent subsequent laparoscopic orchiectomy for testicular nubbins, and 3 underwent subsequent laparoscopic orchiopexy for intracanalicular testes. In two boys with testicular nubbin, an open processus vaginalis was present. Forty-nine boys underwent laparoscopic inguinal ring and/or peritoneal defect closure after orchiopexy or orchiectomy to prevent inguinal hernia formation or hydrocele testis. Concurrent laparoscopic contralateral inguinal closure was performed for two cases with opened contralateral processus vaginalis. Conclusions: Our new strategy is useful because all patients with nonpalpable testis could be treated successfully by only laparoscopic management, avoiding open inguinal incision, preventing inguinal hernia formation, and enabling the concurrent repair of contralateral undescended testis and open processus vaginalis.

Both conventional magnetic resonance imaging (MRI) and ultrasonography (US) have been used for children with non-palpable testes to facilitate a preoperative diagnosis, but the accuracy of these examinations was not satisfactory. In the present study, to improve the diagnostic accuracy of non-palpable testes by MRI examination, we employed fat-suppressed T2-weighted imaging and DWI, as well as conventional MRI. Additional MRI assessments are useful methods to improve the accuracy of diagnosing non-palpable testes. OBJECTIVE To evaluate the magnetic resonance imaging (MRI) findings, diagnostic accuracy and clinical usefulness of fat-suppressed T2-weighted and diffusion-weighted imaging (DWI) in the management of non-palpable testes. PATIENTS AND METHODS In all, 56 children (63 non-palpable testes) aged 8 months-11 years (mean 24.7 months) with non-palpable testes were enrolled. T1- and T2-weighted imaging, as well as fat-suppressed T2-weighted imaging and DWI were performed during MRI examination, and the imaging results were compared with surgical findings. RESULTS In the 63 non-palpable testes, MRI identified 22% (14/63) as intra-abdominal, 20% (13/63) as intra-canalicular, 29% (18/63) as testicular nubbins, and 29% (18/63) as unidentifiable. Although it was difficult to confirm their presence in the abdominal cavity using T1- and T2-weighted imaging alone, additional assessment, including fat-suppressed T2-weighted imaging and DWI facilitated the detection of intra-abdominal testes more easily because they were imaged at a markedly higher signal intensity. The sensitivity, specificity and positive and negative predictive values for the MRI vs the operative findings were 100, 97.3, 96.3, and 100%, respectively. The overall prediction accuracy was 98.4%. CONCLUSION Additional MRI assessments, fat-suppressed T2-weighted imaging and DWI are useful examinations to improve the preoperative diagnostic accuracy of non-palpable testes.
  • Altered Expression and Localization of Estrogen Receptors Alpha and Beta in the Testes of a Cryptorchid Rat Model, Kentaro Mizuno, Yoshiyuki Kojima, Satoshi Kurokawa, Hideyuki Kamisawa, Kenjiro Kohri, Yutaro Hayashi, UROLOGY, 77, (1) 251.e1 - 6,   2011年01月, 査読有り, OBJECTIVES To assess the involvement of estrogen in spermatogenesis, we evaluated the expression of estrogen receptors (ERs) alpha (ERalpha) and beta (ERbeta) in the cryptorchid testes in model rats exposed to flutamide during the fetal stage. METHODS Cryptorchid model rats were produced by administering flutamide to pregnant Sprague-Dawley rats. To evaluate the sequential change in the expression of ERalpha and ERbeta genes, real-time reverse transcriptase-polymerase chain reaction (RT-PCR) was performed using specific primers. Immunohistochemistry with ERalpha and ERbeta antibodies was performed, and the results were evaluated to determine the influences of orchiopexy. RESULTS Real-time RT-PCR revealed that ERalpha expression in control testes increases with growth and peaks value at 7 weeks and significantly decreases in cryptorchid testes. ERbeta expression was low, and there were no significant differences between both the cryptorchid and control testes groups. Immunohistochemistry revealed that ERalpha protein was present in the spermatids and Sertoli cells of descended testes, and that this protein was strongly expressed in the Leydig cells of cryptorchid testes. ERbeta was detected in multiple cells in both groups. After orchiopexy, ERalpha expression was detected in the spermatids of cryptorchid testes. CONCLUSIONS Spermatogenesis in cryptorchid rats is disrupted. Because the expression of ERbeta was unchanged in both control and cryptorchid testes, we supposed that alteration in ERalpha levels is more closely related to spermatogenic failure than ERbeta levels in the cryptorchid testes. We considered that increased expression of ERalpha in Leydig cells of cryptorchid testes is associated with estradiol level in the testicular tissue, and androgen-estrogen imbalance deteriorates spermatogenesis in cryptorchidism. Correcting penile curvature is an essential step in proximal hypospadias surgery. It is logical to lengthen the ventral radius using a graft; however, penile curvature would recur owing to a graft contracture with inadequate vascularization. Although a tunica vaginalis is considered a suitable material to be used as a flap instead of a graft, clinical application of this technique is uncommon. Information regarding the usefulness and effectiveness of this procedure is required. METHODS Between 2003 and 2008, we repaired 146 patients with hypospadias and the urethral plate was transected in 39 patients because of severe curvature. Significant severe curvature persisting even after transection of the plate was corrected with ventral corporeal lengthening procedures using a tunica vaginalis flap in 15 patients and postoperative results were evaluated. RESULTS Straight penile erection was identified in all 15 boys at home by their parents. In 7 of the 15 boys, artificial erection was performed at the time of additional surgery; 5 degrees of curvature was found in 2 patients and 0 degrees in 5. CONCLUSIONS At the second session of surgery, we observed that penile straightening was maintained without recurrence of curvature. Additionally, the area in which the tunica vaginalis flap was placed on the ventrum did not contract. Therefore, lengthening the ventral aspect using a tunica vaginalis flap might be an important alternative for correction of severe penile curvature in hypospadias surgery. UROLOGY 76: 101-106, 2010. (99m)Tc diethylenetriamine pentaacetic acid (DTPA) renography revealed an obstructive pattern in the left kidney, and the existence of a crossing vessel at the UPJ was suspected with excretory urography. Laparoscopic surgery was performed, and intraoperatively no aberrant vessels were found. Upon opening the ureter, a solitary round polyp was found. The segment of the UPJ area containing the polyp was completely removed, followed by laparoscopic dismembered pyeloplasty. Pathologic findings demonstrated a benign fibroepithelial polyp.   • Optimal Cutoff Value of Contralateral Testicular Size for Prediction of Absent Testis in Japanese Boys With Nonpalpable Testis, Yasuhiro Shibata, Yoshiyuki Kojima, Kentaro Mizuno, Akihiro Nakane, Toshiki Kato, Hideyuki Kamisawa, Kenjiro Kohri, Yutaro Hayashi, UROLOGY, 76, (1) 78 - 81,   2010年07月, 査読有り, OBJECTIVES To investigate the accuracy of contralateral testicular hypertrophy in Japanese boys with a nonpalpable testis and to determine the optimal cutoff value of the contralateral testicular size to predict the absence of the testis. METHODS From December 2003 to November 2009, we evaluated 55 boys <60 months old (mean age 19.4) with a unilateral nonpalpable testis and compared their findings with those of a control group. The control group included 20 age-matched boys with 40 testes. The nonpalpable status of the testes was diagnosed by physical examination, and the contralateral testis was measured with calipers before surgery. A sensitivity and specificity analysis were performed using computer software. RESULTS At surgery, the testis was found in 22 of the 55 boys and was absent in 33. Of the 33 boys with an absent testis, 4 had a blind-ending vessel and 29 had an atrophic cord and testicular nubbin in the canal or scrotum. The mean contralateral testicular length and volume in the boys with an absent testis was 22.4 mm and 2.20 cm(3) compared with 16.6 mm and 1.10 cm(3) in the boys with the testis present and 16.6 mm and 1.18 cm(3) in the controls, respectively (P < .01). The predictive accuracy, sensitivity, and specificity for an absent testis was 87.3%, 81.8%, and 95.5% for the length and 85.5%, 84.8%, and 86.4% for the volume, respectively, at the optimal cutoff value of 21 mm in length and 1.6 cm(3) in volume. CONCLUSIONS Our data have shown that contralateral testicular hypertrophy strongly indicates an absent testis in Japanese boys. The optimal cutoff for contralateral testicular hypertrophy with calipers was 21 mm and 1.6 cm(3). This result has provided valuable information for preoperative counseling and treatment planning. In this study, we report the case of a 3-year-old boy who was diagnosed with a hyperplastic intralobar nephrogenic rest extending through the renal pelvis and ureter. After radical nephrectomy, adjuvant chemotherapy was performed as done in stage II Wilms tumor. We also investigated the mutation of the WT1 gene and identified a related single-nucleotide polymorphism (rs16754). We consider that not only various genetic mutations but also single-nucleotide polymorphisms or other epigenetic factors might be involved in the development of Wilms tumor.   • Spermatogenesis After 1-Stage Fowler-Stephens Orchiopexy in Experimental Cryptorchid Rat Model, Hideyuki Kamisawa, Yoshiyuki Kojima, Kentaro Mizuno, Makoto Imura, Kenjiro Kohri, Yuraro Hayashi, JOURNAL OF UROLOGY, 183, (6) 2380 - 2384,   2010年06月, 査読有り, Purpose: To evaluate the validity of 1-stage Fowler-Stephens orchiopexy, we performed this procedure for intra-abdominal testes in an experimental cryptorchid rat model and assessed postoperative spermatogenesis. Materials and Methods: Cryptorchidism in rats was induced by injecting flutamide into the abdomen of pregnant Sprague Dawley rats for 7 days (days 14 to 20 of gestation). Four-week-old cryptorchid rats were divided into the 4 groups of sham operation (group 1, 7 rats), orchiopexy only (group 2, 7), spermatic vessel ligation only (group 3, 7), and spermatic vessel ligation and orchiopexy (1-stage Fowler-Stephens orchiopexy, group 4, 7). The testes were removed 12 weeks after birth in all groups, and testicular weight, testicular histopathological findings and presence of sperm in the epididymis were examined. Results: All operated testes had reticulated vessels on the tunica albuginea. There were no statistically significant differences in the testicular weight between groups 1 (mean +/- SD 0.47 +/- 0.04 gm) and 4 (0.30 +/- 0.19), suggesting that Fowler-Stephens orchiopexy did not induce significant testicular atrophy. However, seminiferous tubule diameters in group 4 were significantly smaller than in group 1 (p <0.001), and no sperm was observed in the epididymis of group 4 rats, suggesting that Fowler-Stephens orchiopexy reduced seminiferous tubule diameters and did not improve spermatogenesis. Conclusions: Although Fowler-Stephens orchiopexy is a good procedure to maintain testicular size and correct cosmetic deformity of an absent scrotum, it may not significantly contribute to the improvement of spermatogenesis.

Medical management has been recommended or preferentially suggested over surgery in all but a few select clinical situations. Prophylactic antibiotics are prescribed routinely in the management of young children with radiological evidence of VUR following an episode of acute pyelonephritis. Prophylaxis is generally maintained until the VUR resolves spontaneously or is corrected surgically. Although the administration of prophylactic antibiotics has been universal in children with VUR, some authors have reported that long-term antibiotic prophylaxis does not fully prevent urinary tract infections or scarring, that antibiotic-related adverse events are known to occur, and that the incidence of pyelonephritis does not increase in spite of prophylactic antibiotic cessation. Recently, four prospective, randomized, controlled trials of antibiotic prophylaxis for preventing pyelonephritis and renal scarring were reported and some placebo-controlled, double-blind prospective studies are ongoing. The goal of this review is to evaluate the treatment of VUR using antibiotic prophylaxis, and its advantages and disadvantages based on appropriate descriptions and studies in the literature.

The shapes of casts and waxed complete dentures were digitally scanned and the extracted shapes of the ridges were uniformly divided both mesiodistally and buccolingually. Principal component analysis was performed using the coordinates of the points on the grid as variables. Over 82% of the variables could be expressed using seven principal components. Methods: Flutamide was injected into the abdomen of pregnant rats for 7 days from the 14th to 20th day of gestation. The male offspring in which cryptorchidism was observed at 28 days after birth were defined as the model rats. They were divided into four groups by dosage of flutamide (2.5 mg, 5 mg, 7.5 mg, 15 mg per day), and their testicular weight, spermatogenesis (modified Johnsen score), and germ cell apoptosis were examined histochemically at 10 weeks after birth. Results: The incidence of cryptorchidism including both unilateral and bilateral in the 2.5, 5, 7.5 and 15-mg flutamide groups was 58.3%, 81.9%, 93.6% and 91.0%, respectively. In the model rats, the undescended testes were located at the caudal end of the abdominal cavity, and these testes weighed less than the contra-descended testes in each group. Histologically, apoptotic cells were markedly increased, the seminiferous tubules were degenerated and disturbance of spermatid differentiation was observed in the undescended testes compared with the normal or contra-lateral descended testes. Conclusions: We found out that the incidence of undescended testes increased in a flutamide dose-dependent manner. The findings of histological examination were independent of the administrated dose of flutamide and it is suggested that exposure of the testes to abdominal temperature causes spermatogenic arrest with germ cell apoptosis. The present animal model indicates high incidence of above 90%, has no surgical stress and dose not require special techniques. We believe that the present model is a useful tool for the understanding of pathogenesis and treatment of cryptorchidism and further biological research into spermatogenesis.

A plain radiograph of his kidney, ureter and bladder and intravenous urography revealed numerous calculi in the upper kidney of his left renal pelvis and ureterocele. A transurethral incision of ureterocele (TUI-ureterocele) and extracorporeal shock wave lithotripsy were performed. On TUI-ureterocele, the many calculi were found to be almost the same size and spherical in form. The postoperative clinical course was uneventful.
  • Assessment of serum follicle-stimulating hormone level and testicular volume for prediction of paternity potential in pubertal boys who underwent bilateral orchiopexy in childhood, Y Kojima, Y Hayashi, K Mizuno, S Kurokawa, S Sasaki, K Kohri, JOURNAL OF UROLOGY, 175, (6) 2290 - 2294,   2006年06月, 査読有り, Purpose: We examined infertile adults for the presence of spermatozoa in the testes, serum FSH level and testicular volume to predict paternity potential in pubertal boys who had undergone bilateral orchiopexy in childhood. Materials and Methods: The study included 58 idiopathic infertile men (group 1), 14 infertile men who had undergone bilateral orchiopexy in childhood (group 2) and 21 pubertal boys who had undergone bilateral orchiopexy in childhood (group 3). We evaluated the correlation between FSH level and testicular volume in all groups. In groups 1 and 2 the testes were examined for the presence of spermatozoa using bilateral testicular biopsy samples. Results: Judging from the presence of spermatozoa in the testes and correlation between FSH level and testicular volume, 14 patients in group 1 (24%) were categorized as being at high risk, 32 (55%) at intermediate risk and 12 (21%) at low risk for loss of paternity potential, with spermatozoa positive rates of 0%, 49% and 100%, respectively. In comparison, 4 patients in group 2 (29%) were categorized as being at high risk, 7 (50%) at intermediate risk and 3 (21%) at low risk, with spermatozoa positive rates of 0%, 43% and 100%, respectively. According to this categorization, 19% of pubertal boys in group 3 were at high risk and may already have lost their paternity potential. Conclusions: Despite bilateral orchiopexy in childhood, some male adolescents may not have the potential for paternity.
  • Mathieu and Barcat repair with a V incision sutured meatoplasty for secondary hypospadias surgery, Y Hayashi, Y Kojima, K Mizuno, A Nakane, T Maruyama, K Kohri, INTERNATIONAL JOURNAL OF UROLOGY, 13, (6) 733 - 737,   2006年06月, 査読有り, Aim: Today single-stage methods are more frequently selected for hypospadias repair than multistaged methods, but complications cannot always be avoided. We employed two kinds of meatal based flip-flap urethroplasty procedures for those with an unsuccessful primary hypospadias repair. Patients and methods: From 1997 to 2005, a meatal based flip-flap urethroplasty was performed as a secondary hypospadias surgery on 17 patients. Basically we attempted the Mathieu repair, but when the wings of the glans were not generously widened and the urethral groove was not sufficiently deep we applied the Barcat procedure. While 11 patients were repaired with the Mathieu technique, six patients underwent the Barcat repair. The V incision sutured meatoplasty was added to obtain a natural ventral slit-like meatus. Results: Ten of the 11 patients who underwent the Mathieu repair had a good outcome, but one patient developed a urethrocutaneous fistula. None of the six patients repaired with the Barcat procedure encountered postoperative complications. Cosmetically, a vertical slit was constructed near the normal neomeatus with the Mathieu and V incision sutured (MAVIS) meatoplasty and the Barcat and V incision sutured (BAVIS) meatoplasty. Conclusions: The Mathieu or the Barcat meatal based flip-flap urethroplasty procedure is feasible as a salvage surgery for those with relatively short urethral defects and adequate mobile ventral skin if an exact procedure is selected. Excellent cosmetic results could be obtained by adding the technique of V incision sutured meatoplasty.
  • Genes associated with the formation of germ cells from embryonic stem cells in cultures containing different glucose concentrations, K Mizuno, A Tokumasu, A Nakamura, Y Hayashi, Y Kojima, K Kohri, T Noce, MOLECULAR REPRODUCTION AND DEVELOPMENT, 73, (4) 437 - 445,   2006年04月, 査読有り, In a previous study, we established a system for visualizing the development of germ cells from mouse embryonic stem (ES) cells in culture using knock-in ES clones in which visual reporter genes were expressed from the mouse vasa homolog, Mvh. While assessing various culture conditions, we found that germ-cell formation was markedly depressed in low glucose medium. Using a repeated polymerase chain reaction (PCR) subtraction method, we identified genes that were differentially expressed in low versus high glucose media. Three genes that were predominantly expressed in high glucose medium, thioredoxin-interacting protein (Txnip), pituitary tumor-transforming gene 1 (Pttg), and RuvB-like protein 2 (RuvB12), were further investigated. These genes were also found to be highly expressed in adult and embryonic gonads, and RuvB12 in particular, which encodes an ATP-dependent DNA helicase, was specifically detected in the spermatocytes and spermatids of the adult testis as well as in primordial germ cells. Furthermore, using a green fluorescent protein (GFP) fusion construct, we found that RuvB12 was expressed in both the nucleus and cytoplasm of testicular germ cells. These findings suggest a possible relationship between glucose metabolism and germ-cell development.
  • Effect of epidermal growth factor on spermatogenesis in the cryptorchid rat, S Kurokawa, Y Kojima, K Mizuno, A Nakane, Y Hayashi, K Kohri, JOURNAL OF UROLOGY, 174, (6) 2415 - 2419,   2005年12月, 査読有り, Purpose: Epidermal growth factor (EGF) is secreted mainly from the submandibular glands. Submandibular gland ablation causes a marked decrease in male fertility, which suggests that EGF influences spermatogenesis. We investigated the effect of EGF in combination with orchiopexy on cryptorchid rat testes in which tubular deterioration had become partially irreversible. Materials and Methods: Unilaterally cryptorchid rats were obtained by daily administration of 7.5 mg flutamide (Nihonkayaku, Tokyo, Japan), an androgen receptor antagonist, to pregnant rats. At age 10 weeks the unilaterally cryptorchid rats underwent orchiopexy with or without EGF administered into the cryptorchid testis. EGF solution (10 mu g/ml) was delivered into the seminiferous tubules by retrograde perfusion through the rete testis. At 14 days testicular recovery was assessed based on the maturity of spermatogenesis using a modified Johnsen score and from the number of apoptotic germ cells per seminiferous tubule. Results: Mean Johnsen score +/- SEM was significantly higher in the orchiopexy with EGF than in the orchiopexy without EGF group (7.85 +/- 0.12 vs 7.12 +/- 0.13, p < 0.001). The number of apoptotic germ cells tended to be smaller in the orchiopexy with EGF group than in the orchiopexy without EGF group (0.16 +/- 0.05 vs 0.28 +/- 0.08). Conclusions: Although orchiopexy for cryptorchidism partly improved spermatogenesis, recovery was limited. EGF administered in combination with orchiopexy was more effective for spermatogenesis than orchiopexy alone. This may be applicable in patients with cryptorchidism.
  • Effect of testicular biopsy in childhood on spermatogenesis, fertility, and paternity in adulthood - A mouse model study, A Nakane, Y Kojima, Y Hayashi, S Kurokawa, K Mizuno, K Kohri, UROLOGY, 66, (3) 682 - 686,   2005年09月, 査読有り, Objectives. To study the effect of testicular biopsy in childhood on spermatogenesis, fertility, and paternity in subsequent adulthood using an animal model. Considerable controversy exists as to whether testicular biopsy should be performed in children. Methods. Four-week-old male ICR mice were subjected to unilateral testicular biopsy (group 1), bilateral biopsy (group 2), or a sham operation (group 3). At 8 weeks of age, the testicular weight, seminiferous tubular diameter, and manifestation of apoptosis were examined. The motility of sperm obtained from the epididymides of the same testes was also examined. Fertility and paternity was assessed in each mouse by housing it with 2 age-matched females for 2 weeks. Results. No significant differences were found in the testicular weight, mean seminiferous tubular diameter, or number of apoptotic germ cells between the biopsied and unbiopsied side in group 1 or between groups 2 and 3. No significant differences were noted in the average sperm motility rate between the biopsied (30.5% +/- 14.5%) and unbiopsied side (35.8% +/- 19.2%) in group 1, or between groups 2 (37.6% +/- 18.0%) and 3 (39.3% +/- 10.0%). All females who coupled with the male mice in all groups became pregnant. The average number of offspring from the 2 female mice was not significantly different among the three groups (group 1, 13.1 +/- 2.3; group 2, 12.3 +/- 2.0; and group 3, 11.3 +/- 3.0). Conclusions. The spermatogenesis, fertility, and paternity of mature mice were unaffected by testicular biopsy in childhood.
  • Molecular evaluation of the SRY gene for gonads of patients with mixed gonadal dysgenesis, K Mizuno, Y Kojima, K Tozawa, S Sasaki, Y Hayashi, K Kohri, INTERNATIONAL JOURNAL OF UROLOGY, 12, (7) 673 - 676,   2005年07月, 査読有り, Aim: To determine whether the SRY gene is present in the gonads of patients with mixed gonadal dysgenesis (MGD). Methods: Molecular analysis was performed in three patients with MGD. Polymerase chain reactions were used to test for the presence of the SRY gene in the peripheral lymphocytes, testes and streak gonads. Results: Chromosome analysis revealed 45,XO/46,XY in two patients, and 46,XY in the third patient. In the peripheral lymphocytes and testicular tissue, the SRY sequences were positive in all cases. However, the SRY sequence was detected in the streak gonad in only two of the three patients. Conclusions: It is interesting that we identified both SRY-positive and SRY-negative streak gonads. Although the SRY gene has a very important role in testicular differentiation, genes other than the SRY gene might also influence the development of the indifferent gonad in MGD.
  • Scrotal dartos flap for the prevention of the urethrocutaneous fistula on hypospadias urethroplasty, Y Hayashi, Y Kojima, S Kurokawa, K Mizuno, A Nakane, K Kohri, INTERNATIONAL JOURNAL OF UROLOGY, 12, (3) 280 - 283,   2005年03月, 査読有り, Background: Urethrocutaneous fistula is a common complication of urethroplasty for severe hypospadias, even when a microsurgical technique is applied. Patients and methods: From June 2001 to July 2003, we applied the scrotal dartos flap wrapping technique to prevent the occurrence of urethrocutaneous fistula in 14 patients with hypospadias. Results: Hypospadias was repaired without fistula formation in all but one patient, who developed a tiny fistula which was later closed successfully by a simple procedure. Conclusions: We recommend the scrotal dartos wrapping technique for covering the neourethra with a well-vascularized flap because of fewer complications, although the procedure to obtain the vascularized flap may take longer.
  • Novel technique for correcting penile curvature with severe hypospadias; Ventral lengthening with tunica vaginalis flap patching, Y Hayashi, Y Kojima, K Mizuno, A Nakane, S Kurokawa, K Kohri, INTERNATIONAL JOURNAL OF UROLOGY, 12, (2) 234 - 238,   2005年02月, 査読有り, The adequate correction of penile curvature is essential for successful hypospadias surgery. We describe a novel technique to correct severe penile curvature with a tunica vaginalis flap on the penile ventrum. We applied a tunica vaginalis flap to lengthen the ventral aspect of the tunica albuginea in two boys with significant curvature and proximal hypospadias. Tunica vaginalis flap patching to the ventral aspect of the penis is safe and technically feasible. If penile curvature is severe or the penis is small in hypospadiac patients, lengthening the ventral aspect using a tunica vaginalis flap is likely to expand instead of dorsal plication or ventral graft.
  • Morphological features of external genitalia in hypospadiac rat model: 3-dimensional analysis, A Uda, Y Kojima, Y Hayashi, K Mizuno, N Asai, K Kohri, JOURNAL OF UROLOGY, 171, (3) 1362 - 1366,   2004年03月, 査読有り, Purpose: We examined 3-dimensionally the process of external genitalia formation in the experimental hypospadiac rat model. Materials and Methods: We administered 7.5 mg flutamide daily, a blocker of androgen receptor, into the abdomen of naturally pregnant female Sprague-Dawley rats from gestational days 14 to 20 to produce a hypospadiac rat model. The control group consisted of male offspring not exposed to flutamide. The fetal phallus was obtained at gestational days 17.5, 19.5 and 21.5. We observed them by scanning electron microscopy. Results: In the 17.5-day-old embryo the projection on the ventral side of the phallus was observed from the base of the phallus to the coronary sulcus in the control rat. This finding demonstrated that the urethra develops from the base of the phallus to the coronary sulcus. On the other hand, this projection was not observed in the hypospadiac rat and the urethra was not seen on the ventral side of the phallus. In the 19.5-day-old embryo the ventral preputial closure appeared in the proximal phallus of the control rat but not of the hypospadiac rat. In the control rat the scrotum was discerned in the perineum and divided by a median fold (the raphe). The raphe reached the base of the phallus, where it was concurrent with the preputial fold that covers the urethra. On the other hand, the hypospadiac rat did not have a raphe and the hollow at the base of the phallus appeared to give rise to the future orifice of the urethra. In the 21.5-day-old embryo the control rat prepuce completely surrounded the distal. phallus, whereas that of the hypospadiac rat was dorsally hooded with a ventral cleft and the external appearance resembled the morphology of human penoscrotal hypospadias. Conclusions: This hypospadiac rat model is considered valuable for further studying penile growth and differentiation, and the molecular mechanisms in external genitalia formation in hypospadias.

To our knowledge, this is the fourth case of its kind to be reported in the literature. Ultrasonography, magnetic resonance imaging and biopsy were useful for making an accurate diagnosis in this case. The diagnosis was confirmed by suprapubic transcutaneous needle biopsy. The tumor was resected approximately 3 years after diagnosis, during which period the patient delivered a normal baby by caesarian section.
  • Modified technique of dorsal plication for penile curvature with or without hypospadias, Y Hayashi, Y Kojima, K Mizuno, K Tozawa, S Sasaki, K Kohri, UROLOGY, 59, (4) 584 - 586,   2002年04月, 査読有り, Objectives. Since Nesbit reported dorsal plication at the lateral aspect of each corpus cavernosum to straighten the bending penis, dorsal plication on each side of the penis at the 2-o'clock and 10-o'clock positions has been a mainstay for correction of congenital or acquired penile curvature. However, because only the 12-o'clock position proved to be the nerve-free zone as determined by the histologic research, Baskin advocated that dorsal plication be done at the 12-o'clock position. Methods. We performed plication at the 12-o'clock position of the corporeal body to repair penile curvature with and without hypospadias. We modified Baskin's plication technique, approximating the outer edges of the incisions made through the tunica albuginea. Results. We repaired penile curvature with hypospadias in 10 patients and penile curvature without hypospadias in 3 patients, without complications. Conclusions. Penile curvature due to corporeal disproportion could be repaired by dorsal plication with incisions of the tunica at the 12-o'clock position, irrespective of the presence or absence of hypospadias.
  • Spermatogenesis, fertility and sexual behavior in a hypospadiac mouse model, Y Kojima, Y Hayashi, K Mizuno, M Mogami, S Sasaki, K Kohri, JOURNAL OF UROLOGY, 167, (3) 1532 - 1537,   2002年03月, 査読有り, Purpose: Administering of flutamide to pregnant mice causes hypospadias in male offspring. We investigated spermatogenesis, fertility and sexual behavior in this hypospadiac mouse model. Materials and Methods: Male offspring exposed to flutamide during the embryonic period were divided into hypospadiac group 1 and normal external genitalia group 2. Control group 3 consisted of male offspring not exposed to flutamide. We analyzed the, spermatogenesis, epididymides sperm motility, in vitro fertilization rate and sexual behavior of each mouse. Results: There were no significant differences in the weight of the testes or mean seminiferous tubular diameter in the groups. The number of apoptotic germ cells per unit area was not significantly different in the 3 groups. In groups 1 to 3 there were no significant differences in the mean epididymides sperm motility rate plus or minus standard deviation (62.6% +/- 10.0%, 57.2% +/- 7.0% and 67.0 +/- 7.6%) or in the in vitro fertilization rate (52%, 48% and 48%, respectively). However, there were significant differences in groups 1 to 3 in mean mounting frequency (0, 29 +/- 4.0 and 12.4 +/- 4.5 times per hour) and mean intromission frequency (0, 24.4 +/- 3.5 and 3.8 +/-1.5 times per hour, respectively). Females coupled with group 1 or 2 male mice did not achieve pregnancy. Conclusions: These results suggest that spermatogenesis, sperm motility and fertilization in vitro were unaffected in hypospadiac mice but sexual motivation and arousal were deficient.
  • Tubularized incised plate urethroplasty for proximal hypospadias, K Mizuno, Y Hayashi, Y Kojima, K Tozawa, S Sasaki, K Kohri, INTERNATIONAL JOURNAL OF UROLOGY, 9, (2) 88 - 90,   2002年02月, 査読有り, Objectives: Numerous surgical procedures have been used to correct distal hypospadias. Among them, the tubularized incised plate urethroplasty (Snodgrass procedure) has become a mainstay for the repair of distal hypospadias. We applied the procedure to proximal hypospadias. Methods: Three patients with proximal hypospadias underwent a tubularized incised urethral plate urethroplasty. The location of the meatus was proximal penis in one, penoscrotal margin in one and scrotum in one. A perimeatal incision was made and the two paramedian incisions were extended to the tip of the glans. The skin of the penile shaft was dissected free to the penoscrotal junction and bands of fibrous tissue were excised until the corpus spongiosum proximal to the meatus was completely exposed inside the scrotum. The urethral plate was then incised in its midline from the tip of the glans to the hypospadiac meatus and was tubularized without tension. The neourethra was covered with a pedicle of subcutaneous tissue dissected from the dorsal skin or the scrotal skin to avoid fistula formation. Results: The tubularized incised urethral plate urethroplasty was carried out successfully in one stage on three patients with proximal hypospadias. Conclusions: The Snodgrass procedure is suitable for correcting hypospadias in patients with a healthy urethral plate. It is also suitable in patients with proximal hypospadias.

  • 【小児腹腔内精巣-どう扱うか?】腹腔鏡下精巣固定術 精巣血管の温存をめざして, 水野 健太郎, 西尾 英紀, 守時 良演, 加藤 大貴, 安井 孝周, 林 祐太郎, Japanese Journal of Endourology, 31, (1) 37 - 43,   2018年04月, 停留精巣の20〜35%は非触知であり、今日では診断・治療を目的として広く腹腔鏡手術が行われるようになった。私たちは非触知精巣に対して原則として全例で臍部からの腹腔鏡視を行い、消失精巣に対しても腹腔鏡のみで完遂するアルゴリズムで対応している。腹腔内精巣に対しては、精巣血管を遮断するFowler-Stephens法ではなく、できるだけ精巣血流を温存して精巣固定術を行う工夫を行ってきた。この理由として、精巣血流を遮断するFowler-Stephens法では、1.6〜5.5%の症例に精巣萎縮が見られることや、精巣発育についての長期成績が明らかでなく、症例数が限られているため十分なRCTが行われていないことが挙げられる。また、私たちが確立した停留精巣動物モデルを用いた基礎研究で、精巣血流の遮断によって精子形成が高度に障害されたことから、血流温存が精巣の機能維持に重要と考えられたことも、その理由の一つである。腹腔内精巣の血流を温存して精巣を下降させるために私たちは、(1)精巣血管の可及的頭側への剥離、(2)精巣血管の腹膜からの剥離、(3)trans scrotal trocarの利用、(4)良好な視野確保、といった点に注意して手術を行っている。本稿では、腹腔内精巣に対する腹腔鏡下精巣固定術において、私たちが行っている実際の手術方法を述べるとともに治療成績や問題点についても概説する。(著者抄録)

  • 膀胱尿管逆流術後の尿中N-アセチル-β-D-グルコサミニダーゼ高値持続と分腎機能低下, 丸山 哲史, 西尾 英紀, 守時 良演, 加藤 大貴, 神沢 英幸, 黒川 覚史, 中根 明宏, 水野 健太郎, 林 祐太郎, 安井 孝周, 日本小児泌尿器科学会雑誌, 26, (1) 47 - 54,   2017年06月, 【目的】逆流性腎症を早期に予測することを目的とし、急性腎不全バイオマーカーの術後経過と腎機能経過との関連性を検討した。【方法】対象は逆流防止術後に尿路感染なく10年以上経過した38例。Dimercaptosuccinic acid(DMSA)腎シンチグラフィーを術前および術後1、3、5年で施行し、患側の相対摂取率で分腎機能を評価した。血中クレアチニン、尿中N-アセチル-β-D-グルコサミニダーゼ(NAG)、尿中α1マイクログロブリン(AMG)、血中レニン及びアルドステロンを術後継続的に測定した。【結果】尿中NAGは術後に26/29例(90%)で異常高値を認めたが、長期経過中に23例(80%)で改善した。高度逆流群の術後初回分腎機能改善率は6.8±9.2%で、軽度逆流群の-1.1±4.3%に比較し有意に高値であった(p=0.0365)。2例(7%)において尿中NAGの低値にもかかわらず長期経過中に分腎機能が低下した。血中クレアチニンは3例で高値、尿中AMGは1例、血中レニン及びアルドステロンは1例で高値であった。【結論】逆流防止術後に尿中NAGは高値持続したが徐々に改善し、可逆的な慢性炎症状態であったと推察された。今回の検討では尿中NAGの慢性腎不全予測マーカーとしての有効性は明らかではなかった。高度逆流を認めた症例においては逆流防止術による逆流圧消失効果が示唆された。(著者抄録)
